Evaluation of the impact of early childhood caries on oral health-related quality of life for Turkish preschool children and their families

Abstract

Amaç: Çalışmamızın amacı erken çocukluk çağı çürükleri grubu (EÇÇ) ile kontrol grubu karşılaştırarak erken çocukluk çağı çürüklerinin okul öncesi çocuklar ve aileleri için ağız sağlığı ile ilgili yaşam kalitesi (OHRQoL) üzerindeki etkisini değerlendirmektir. Yöntem: 2-5 yaş arasındaki çocuklara kesitsel bir anket yapıldı. Çocuklar EÇÇ grubu ve kontrol grubu (çürük olamayan) olarak iki gruba ayrıldı. Genel sorular, Erken Çocukluk Çağı Ağız Sağlığı Etki Ölçeği’nin (T-ECOHIS) ve çocuklarda ağız sağlığı ile ilgili yaşam kalitesinin Türkçe versiyonu (POQL) anketleri ebeveynler tarafından dolduruldu. Çürük, eksik ve dolgulu dişler’in (dft) değerleri DSÖ kriterlerine göre tespit edildi. Gruplar arasındaki farklar, p<0,05’te spesifik önem düzeyi ile Chi-square ve Mann-Whitney U testi kullanılarak değerlendirilmiştir. Bulgular : Üç yüz çocuk çalışmaya katıldı. EÇÇ grubu ve kontrol grubu 150 erkek (%50) ve 150 kız (%50) olarak oluşturuldu. EÇÇ ve dft kontrol grupları arasında anlamlı bir fark bulundu (p=0,0001). T-ECOHIS (Çocuk ve Aile Etki Bölümleri) için EÇÇ Grubu skorları kontrol grubundan anlamlı olarak daha yüksek bulundu (p=0,0001). Tüm POQL bölümlerinde, EÇÇ grup skorları kontrol grubundan anlamlı olarak daha yüksek bulundu (p=0,0001). Sonuç: EÇÇ’nin okul öncesi çocuklar ve ailelerinin OHRQoL üzerinde yüksek derecede olumsuz bir etkisi vardır. Bunun için EÇÇ’yi önlemek ve kontrol etmek için çeşitli tedbirler alınmasına gerek duyulmaktadır.

Objective: To compare the Early Childhood Caries (ECC) group to the Control group (caries-free) to assess the effect of ECC on the Oral Health-Related Quality of Life (OHRQoL) of preschool children and their families. Methods: A cross-sectional survey was conducted on children ages 2–5. Children were classified into two groups, the ECC group, and the Control group. A survey has been used consisting of general questions, the Turkish version of the Early Childhood Oral Health Impact Scale (T-ECOHIS), and the Turkish version of Pediatric Oral Health-Related Quality of Life (POQL).Surveys filled by parents. The decayed, missing, and filled teeth (dft) were detected according to WHO criteria. Differences between the groups were evaluated using Chi-square and Mann–Whitney U test with a significance level set at p<0,05. Results: Three hundred children were enrolled. The ECC group and Control group consisted of n=150 boys (50%), and n=150 girls (50%). There was a significant difference between the ECC and Control groups in dft (p=0,0001). For T-ECOHIS (The Child and Family Impact Sections), scores of the ECC group were significantly higher than the Control group (p=0,0001). In all sections of POQL, the scores of the ECC group were significantly higher than the Control group (p=0,0001). Conclusion: ECC has a great negative impact on OHRQOL on preschool children and their families. This implies the need for measures to prevent and control ECC.
