Pediatrik rehabilitasyonda zorunlu kullanım hareket terapisinin yeri nedir?

Abstract

Zorunlu kullanım hareket terapisi (ZKHT) hemiparetik hastalarda üst ekstremite (ÜE) fonksiyonunu iyileştirmede ümit verici rehabilitasyon yöntemlerinden biridir. Temel ZKHT protokolü en az etkilenmiş ÜEnin kısıtlanmasını, etkilenmiş ÜEye uygulanan 2-3 hafta süreli yoğun, tekrarlayan motor egzersiz tedavisini ve şekillendirme terapisini içermektedir. Şimdiye kadar serebral palsi, inme, travmatik beyin yaralanması ve obstetrik brakial pleksus paralizisi tanılı çocuklarda ZKHT ile ilgili birçok olgu serisi, randomize kontrollü çalışma yayınlanmıştır. Protokoller kullanılan farklı kısıtlama yöntemleri, egzersiz yoğunluğu ve süresi, değerlendirme ölçütleri nedeniyle farklılıklar göstermektedir. Bu makalede ZKHTnin hemiplejik ÜE fonksiyonunu geliştirmekteki etkinliğini araştıran çalışmalar ve bu yeni tedavi yönteminin çocuk hastalarda kullanımı gözden geçirilecektir. Constraint-induced movement therapy (CIMT) is one of the promising rehabilitation interventions for improving the function of the upper extremity (UE) in patients with hemiparesis. Basic CIMT protocol includes constraining the use of the less-impaired UE, intensive, repetitive practice of motor training with the impaired UE for 2 3 weeks, and shaping therapy. So far numerous case studies, randomized controlled trials have been reported about CIMT in children with cerebral palsy, stroke, traumatic brain injury and obstetric brachial plexus paralysis. Protocols vary widely in terms of type of constraint used, intensity and duration of training, and outcome measures. This article will review studies evaluating the efficacy of CIMT in improving the hemiplegic arm functioning and the application of CIMT in pediatric patients. Turk J Phys Med Rehab 2013;59:250-5.
