Publication: Obez infertil erkeklerde INHBB geni fenotip ilişkisi
Abstract
Dünya nüfusunun % 15’ini etkileyen kısırlıkta erkek faktörü, çiftlerin yaklaşık %50’sini etkilemektedir. Erkek infertilitesinde semene ait anormallikler genellikle testislerde sperm üretiminde problem nedeniyle ortaya çıkmaktadır. Son yıllarda artmış vücut kitle indeksinin semen parametrelerine olumsuz etki ettiğini gösteren çalışmalar yapılmıştır. Gelişmiş ve gelişmekte olan ülkelerde en büyük sağlık problemlerinin başında gelen obezitenin prevalansı son yıllarda gözle görülür bir şekilde artmış, bu artış; metabolik, endokrin ve yapısal sorunları da beraberinde getirmiştir. Aşırı yağlanma durmunda oluşan endokrin problemler, yeterli salgılanamayan hormonlar ve bozulmuş spermatogenez, obezitenin erkek infertilitesiyle ilişkisini göstermektedir. Son yıllarda yapılan çalışmalarla spermatogenez belirteci olarak belirlenen inhibin B hormonu, sertoli hücrelerinden salgılanarak FSH ile ters etkili olarak çalışır. FSH gibi hipofiz tarafından dolaylı olarak değil, direkt olarak testislerden salındığı için spermatogenezin önemli bir göstergesi olarak kabul edilir. Bu çalışmanın amacı, obez infertil erkeklerde inhibin B hormonu düzeylerini ve bu hormonu kodlayan gen olan INHBB genini incelemektir. VKİ 30’un üstünde ve sperm sayıları 20 milyon altında olan 37 obez, kısır erkekte inhibin B hormonu düzeyleri Elisa yöntemiyle ölçüldü. Genomik DNA izolasyonu yapılarak INHBB genindeki Ekzon 1 ve Ekzon 2 bölgeleri PZR yöntemiyle çoğaltıldı. Gendeki olası varyasyonları belirlemek amacıyla dizi analizi yapıldı. 37 obez infertil erkekten elde edilen örnek sonuçlarına göre anormal inhibin B hormonu düzeylerine veya INHBB geninde herhangi bir varyasyona rastlanmadı. nfertilite, Obezite, İnhibin B, INHBB
Relationship Between INHBB Gene And Phenotype In Obese Infertile Men Male factor in infertility which affects 15 % of people in the world, affects nearly 50 % of infertile couples. In male infertility, abnormalities relating to semen generally occur due to the sperm production problem in testicles. There has been recent studies showing the negative effect of increased body mass index on semen parameter. In both developed and developing countries obesity prevalence, which comes first among the biggest health problems, has increades dramatically in recent years. This increase brings itself with metabolic, endocrine and structural problems. Endocrine problems, hormones that are not secreted enough and degenerative spermatogenesis all of which occur in the event of being overweight shows the realtion between obesity and male infertility. In recent studies inhibin B hormone, identified as spermatogenesis marker, works as negative feedback with FSH by being secreted from sertoli cells. It is accepted as an important marker of spermatogenesis since unlike FSH which is secreted indirectly by pituitary, it is secreted directly from testicles. The purpose of this study is to examine inhibin B hormone levels in obese infertile males and INHBB gene which codes this inhibin B hormone. Inhibin B hormone levels in 37 obese and infertile males whose BMI is over 30 and sperm count is under 20 million was measured by Elisa method. By doing Genomic DNA isolation, Exon 1 and Exon 2 parts in INHBB gene were amplified by PCR method. With the aim of identifying potential variations in the gene, sequencing was done. According to the sample results obtained from 37 obese infertile males, inhibin B hormone levels or any variations in INHBB gene weren’t encountered. Key Words: infertility, obesity, inhibin B, INHBB
