Kronik B Hepatit'lerde Steatoz/ Steatohepatit'in stellat hücre ve progenitor hücre dağılımına etkisi : Hepatit B virüsü negatif steatohepatit olgularla karşılaştırılması

Abstract

Amaç: Çalışmamızda, Kronik Hepatit B (KH-B) olgularında steatoz ve steatohepatit varlığının karaciğerde myofibroblastik farklılaşma gösteren stellat hücre yoğunluğu ve dağılımı üzerine etkisini araştırmak ve fibrogenez sürecinde CD56 pozitif NK hücrelerin rolünü belirlemek amaçlanmıştır. Gereç ve Yöntem: 2011-2020 yıllarında bölümümüzde KH-B tanısı alan karaciğer (KC) biyopsilerinden 22 KH-B steatoz (-), 20 KH-B steatoz (+) ve 22 KH-B steatohepatit (+) olgudan üç grup oluşturulup, 22 saf steatohepatit olgudan kontrol grubu seçildi. Çalışmaya alınan olgulara trikrom, retikülin ve SMA, CD56 İHK boyaları uygulanarak, dağılım ve boyanma yoğunluğu semikantitatif olarak değerlendirildi. SMA ve CD56 dağılım ve yoğunluğunun, gruplar arasındaki değerleri, yağlanma derecesi, balonlaşma derecesi ve fibrozis skoru ile ilişkisi Fischer exact test, SMA ile CD56‟nın dağılım ve yoğunluk açısından karşılaştırılması Spearman korelasyon testi ile analiz edildi. Bulgular: KH-B steatoz (-) grubunda SMA ile boyanma saptanmadı; dağılım ve yoğunluk açısından bu grup ile diğer gruplar arasında anlamlı fark belirlendi; ancak diğer grupların kendi aralarında anlamlı fark yoktu. KH-B steatoz (-) grubunda CD56 ile boyanma yalnız 10 olguda ve zon-1‟de sınırlı saptandı; dağılım ve yoğunluk açısından bu grup ile KH-B steatoz (+) arasında fark saptanmamasına karşın diğer gruplarla arasında anlamlı fark belirlendi. Diğer grupların birbirleri arasında CD56 boyanma yoğunluğu açısından fark yoktu, ancak zonal dağılım farkı belirgindi (p<0,01). Yağlanma yoğunluğu ve balonlaşma derecesi CD 56 yoğunluk ve dağılımı ile, fibrozis skoru CD56 dağılımı ilişkili bulundu. SMA ve CD56 dağılımları ve yoğunlukları arasında korelasyon izlendi (p<0,01). Sonuç: Çalışmamız, özellikle steatohepatit izlenen KH-B olgularında aktif fibrogenez sürecinin devam ettiğini ve bunun CD56 pozitif NK hücreleri ile ilişkili olabileceğini göstermektedir.

Objective: Our study aimed to investigate the effect of the presence of steatosis and steatohepatitis on the density and distribution of stellate cells showing myofibroblastic differentiation in the liver in Chronic Hepatitis B (CH-B) cases and to determine the role of CD56 positive NK cells in the fibrogenesis process. Materials and Methods: Liver biopsies from 86 CH-B cases (22 without steatosis, 20 with steatosis, and 22 with steatohepatitis) were analyzed for trichrome, reticulin, SMA, and CD56 immunohistochemistry. The density and distribution of SMA+ stellate cells and CD56+ NK cells were assessed and correlated with histological features, including steatosis, ballooning degeneration, and fibrosis score. Results: In the CH-B steatosis (-) group, SMA staining was absent, while CD56 staining was confined to zone 1 in a subset of cases. Significant differences in CD56 distribution and intensity were observed between steatosis (-) and other groups (p<0.01), with notable correlations between CD56 distribution and both fat density and ballooning degeneration. The presence of CD56+ NK cells was further associated with increased fibrosis. A positive correlation between SMA and CD56 staining was found (p<0.01), suggesting an interrelation between stellate cell activation and NK cell presence in fibrogenesis. Conclusion: Our study shows that the active fibrogenesis process continues, especially in CH-B cases with steatohepatitis, and this may be associated with CD56 positive NK cells.
