Total kalça protezi uygulanan hastalarda immobilizasyonun hasta üzerine etkisi

Abstract

Araştırma; Total kalça protezi uygulanan hastalarda immobilizasyonun hasta üzerine etkilerini incelenmek ve bu etkilerin sonucunda hastayı holistik bakış açısıyla değerlendirmek amacı ile hastalara anket, izlem formları ve oxfort kalça skoru uygulanması şeklinde yapılmıştır. Araştırmada 15 Eylül 2006 – 5 Eylül 2007 tarihleri arasında Marmara Üniversitesi Hastanesi ortopedi ve travmatoloji servisi (44 hasta), Lütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi ortopedi 1 (41 hasta) ve ortopedi 2 servisi’nde (41 hasta) elektif ve acil olarak başvuran toplam 126 hasta çalışma kapsamına alınmıştır. Başvuran hastalardan total kalça protezi ameliyatı uygulanan, bilinci açık olan tüm yaş grupları çalışma kapsamına alınmıştır. Araştırmada 126 hastaya hastaya Tanılama Formu, Oxfort Kalça Skoru İmmobil Hasta İzlem Formu, Laboratuvar Bulguları İzlem Formu ve Vital Bulguların İzlem Formu uygulanmıştır. Oxfort Kalça Skoru post-op 3 ay sonra tekrar uygulanmıştır. Araştırmada total kalça protezi uygulanan hastalarda immobilizasyonun fizyolojik etkilerinden; konstipasyon, iştahsızlık, ortostatik hipotansiyon, hemoglobin seviyesinde azalma ve bası ülserleri görüldüğü belirlenmiştir. Psikolojik etkileri incelendiğinde ise anksiyete, deliryum, depresyon ve uyku bozukluklarında belirgin bir artış olduğu görüldü. Total kalça protezi ameliyatı olan hastalarda ameliyat öncesi ve ameliyattan 3 ay sonra uygulanan oxfort kalça skorunun puanlarında azalma olduğu görüldü. EFFECTS OF İMMOBİLİZATİON ON THE PATİENTS WİTH TOTAL HİP PROTHESİS The research was conducted with the purpose of analyzing the effect of immobilization on the patients with hip prothesis and as a result of these effects, of evaluating the patients with a holistic point of view through applying questionnaire observation forms and Oxford hip score to the patients. Total 126 patients consisting in the ones that effectively and in emergeny applied to Marmara University Hospital Orthopedics and Traumatology Service (44 patients), Lütfi Kırdar Kartal Education and Research Hospital Orthopedics Service 1(41 patients), Orthopedics Service 2 (41 patients) between the dates September 15, 2006-September 5, 2007 were included in scope of the research. The patients from all groups who were concious, were included in scope of the research. Within the research, data were gathered trough appying İdentification Form, Oxford Hip Score, İmmobile Patient Observation, Laboratory Findings Observation Form and Vital Findings Observation Form to 126 patients. Oxford Hip Score was applied again 3 months after post-operation. With the research it, was determined that the patients with total hip prothesis experience physiological effects of immobilization such as constipation, orthostatik hypotension, decrease in hemoglobin level and pressure ulcer. When its psychological effects were analyzed, it was seen that there was an apparent increase in the signs of anxiety, delirium, depression and sleeping disorders. It was seen that there had been a decrease in the points of oxford hip score that had been applied before and 3 months after operation. Key Words: Total Hip Prothesis, İmmobilization, Oxfort Hip Score
