Abstract

Hematoloji Kliniğinde Akut Solunum Yetmezliği Gelişen Hastalara Uygulanan Erken Dönem Noninvaziv Solunum Desteğinin EtkisiÖğrencinin Adı: Zeynep Gök EKİNCİDanışmanı: Prof. Dr. Mine Gülden POLATAna Bilim Dalı: Kardiyopulmoner Fizyoterapi ve Rehabilitasyon Anabilim DalıÖZETAmaç: Çalışma; hematoloji kliniğinde akut solunum yetmezliği gelişen hastalara uygulanan erken dönem noninvaziv solunum desteğinin, fizyolojik parametrelere ve mortalite üzerine etkisinin değerlendirilmesi amacıyla gereçekleştirildi.Gereç ve Yöntem: Çalışmaya hematoloji kliniğinde izlem sırasında akut solunum yetmezliği gelişen hastalar dâhil edildi. Akut solunum yetmezliğinin başladığı gün, hastalar yoğun bakım ünitesine alınmadan solunum fizyoterapisti tarafindan noninvaziv solunum desteği verilmeye başlandı (mod: BIPAP-ST). Hastaların akut solunum yetmezliği parametreleri günlük takip edildi. Yoğun bakım endikasyonu gelişen hastalar yoğun bakım ünitesine yönlendirildi.Bulgular: Çalışmaya 5 kadın, 8 erkek olmak üzere 13 (yaş ort 63,0±.7 yıl) hasta dâhil edildi. Noninvaziv solunum desteğinde başarı sağlanamayan olgularda mortalite oranı daha yüksek bulundu (p=0,02). Olguların; APECHE 2 skorlarına göre beklenen ölüm oranları değerlendirildiğinde beklenen ölüm oranından daha yüksek bir gerçekleşen ölüm oranı görülmedi. Ancak noninvaziv solunum desteğinde başarılı olunan ve olunmayan gruplar incelendiğinde, otolog nakillerde ve kemik iliği nakli yapılmamış olgularda başarı oranının daha yüksek olduğu (otolog nakil %83,33, allojenik nakil %0, kemik iliği nakli yapılmamış olgular %75) görüldü.Sonuç: Hematolojik maligniteli hastalarda akut solunum yetmezliği bir mortalite nedenidir. Erken dönemde solunum yetmezliği belirtileri gözlenirse erken dönemde uygulanan noninvaziv solunum desteğinin sonuçları iyileştirici etkisi vardır.

The Effect of Early Noninvasive Respiratory Support for The Patients with Acute Respiratory Failure in the Hematology ClinicStudent's Name: Zeynep GÖK EKİNCİConsultant: Prof. Dr. Mine Gülden POLATDepartment: Cardiopulmonary Physiotherapy and Rehabilitation DepartmentABSTRACTPurpose: The purpose of this study was to evaluate the effect of early noninvasive respiratory support on mortality in patients with acute respiratory failure in hematology clinic.Material and Method: Patients who developed acute respiratory failure during follow-up in hematology clinic were included in the study. On the day acute respiratory failure began, noninvasive respiratory support was given by the respiratory physiotherapist without admitting the patients to the intensive care unit (mode: BIPAP-ST). The parameters of acute respiratory failure of the patients were monitored daily. The patients who developed ICU indication were referred to the ICU.Findings: The study included 13 patients, of whom 5 were female and 8 were male (average age 63.0±.7 years). Mortality rate was found to be higher in cases with unsuccessful noninvasive respiratory support (p=0.02). Of the cases; When the expected mortality rates were evaluated according to APACHE II scores, higher mortality rates than expected were not observed. However, when successful and unsuccessful groups with noninvasive respiratory support were examined, the success rate was higher in autologous transplantation and in cases without bone marrow transplantation (autologous transplantation 83.33%, allogeneic transplantation 0%, bone marrow transplantation 75%).Conclusion: Acute respiratory failure is a cause of mortality in patients with hematologic malignancies. If early signs of respiratory failure are observed, the results of early noninvasive respiratory support have curative effects.
