Abstract

1.ÖZET Normal ve Sensorinöral İşitme Kayıplı Yetişkin Bireylerde ASSR (İşitsel Sürekli Durum Cevabı) ile Elde Edilen İşitme Eşiği Seviyelerinin Tonal Uyaranlı İBC (İşitsel Beyinsapı Cevabı) ve Saf Ses İşitme Eşikleri ile Karşılaştırılması Ayşe Neşe Kiraz, Prof. Dr. Ferda Akdaş Odyoloji ve Konuşma Bozuklukları Bilim Dalı Bu araştırmanın amacı, normal ve sensorinöral işitme kayıplı yetişkin bireylerde chirp uyaran kullanarak elde edilen ASSR eşikleri, İBC ve saf ses işitme eşikleri ile karşılaştırılması ve böylece ASSR testinin işitme eşiğini yordamadaki güvenirliliğinin değerlendirilmesidir. Araştırmaya beş grup katılımcı dahil edilmiştir. Birinci grup; normal işiten 15 yetişkinden ve diğer dört grup ise farklı derecelerde farklı odyogram konfigürasyonuna sahip 40 sensorinöral işitme kayıplı yetişkinden oluşmaktadır. Çalışmaya katılan kişilere akustik immitansmetri, DPOAE ve saf ses odyometrisi uygulanmış, daha sonra Interacoustic Eclipse platformunda ASSR ve İBC testleri kullanılarak 500, 1000, 2000, 4000 Hz’ de işitme eşikleri tespit edilmiştir. ASSR ve İBC testlerinde, kişinin rahat bir pozisyonda uzanması ve uyuması istenmiştir. İşitme eşiklerinin karşılaştırılması için Pearson korelasyon analiz yöntemi uygulanmıştır. Normal ve sensorinöral işitme kayıplı bireylerden elde edilen ASSR, İBC ve saf ses işitme eşik ortalamaları birbirleriyle karşılaştırılmıştır. Normal işitenlerde, ASSR eşiklerinin İBC eşiklerine kıyasla, saf ses işitme eşikleriyle daha uyumlu olduğu görülmektedir. İşitme kayıplı gruplarda; saf ses işitme eşikleri, İBC eşikleri ve ASSR eşiklerinin ortalama değerleri birbirine yakın elde edilmiştir. Sonuç olarak, chirp uyaran kullanarak elde edilen ASSR eşiklerinin hem normal işitenlerde hem de işitme kayıplı bireylerde saf ses işitme eşiklerine yakın sonuçlar vermesi, ASSR testinin işitme eşiklerini belirlemede güvenilir olarak kullanılabilecek bir test olduğunu göstermektedir. Comparing ASSR (Auditory Steady State Response) Hearing Thresholds to Tonal Evoked ABR (Auditory Brainstem Response) and PTA (Pure Tone Audiometry) Thresholds in Adults with Normal and Sensorineural Hearing Loss Ayşe Neşe Kiraz, Prof. Dr. Ferda Akdaş Department of Audiology and Speech Pathology The aim of the study is to compare ASSR thresholds obtained using chirp stimulus with ABR and PTA thresholds in adults with normal and sensorineural hearing loss and to evaluate the reliability of ASSR thresholds for the prediction of the auditory thresholds. Five groups of participants were consisted in this study. The first group of 15 adults with normal hearing and the other four groups of different audiogram configurations with varying degrees of 40 sensorineural hearing loss adults was comprised. Acoustic immitansmetri, DPOAE, pure tone audiometry were performed on all subjects, then ASSR and ABR thresholds were obtained at both ears at 500, 1000, 2000, 4000 Hz in Interacoustic Eclipse platform. Before ASSR and ABR testing, participants were asked to lying down and sleeping in a comfortable position. Pearson correlation analysis method was used to evaluate the relationship with hearing thresholds. In five groups; the mean differences between ASSR thresholds, ABR thresholds and pure tone thresholds for different frequencies were assessed. Groups with normal hearing, ASSR thresholds and ABR thresholds were compared with pure tone thresholds, pure tone thresholds were seen to be more compatible with ASSR thresholds. In the groups with sensorineural hearing loss; the mean differences of pure tone thresholds, ABR thresholds and ASSR thresholds ​​were closed to each other. As a result, ASSR thresholds obtained by using chirp stimulus are congruent with pure tone audiometry with normal hearing and sensorineural hearing loss groups, therefore ASSR test is a reliable test for accurate prediction of auditory thresholds.
