Publication: Epilepsi hastalarında antiepileptik ilaç tedavisinde uyumun değerlendirilmesi
Abstract
Çalışma; epilepsili yetişkin bireylerin hastalık hakkında bilgi düzeylerini, hastalığı algılayış şeklini, antiepileptik ilaç tedavisine uyumu etkileyen faktörleri değerlendirerek, epilepsi hastalarının sağlık bakım gereksinimlerinin planlanmasında rehber olmak ve ülkemizin epilepsi hemşireliğine olan gereksinimini belirtmek amacıyla yapıldı. Çalışma 01.09.2005-01.03.2006 tarihleri arasında Gülhane Askeri Tıp Akademisi Haydarpaşa Eğitim Hastanesi nöroloji polikliniklerine başvuran 205 epilepsili hasta ile gerçekleştirildi. 18 yaş üstü, okur yazar, çalışmaya katılmayı kabul eden, en az iki yıldır epilepsi tanısıyla izlenen, antiepileptik tedavi alan, psikiyatrik sorunu olmayan tüm hastalar çalışmaya dahil edildi. Veri toplama aracı olarak literatür bilgisine dayalı hazırlanan hasta görüşme formu kullanıldı. Bu form; hastaların hastalık hakkında bilgi düzeylerini, sosyodemografik özelliklerini, hastalıkla ilgili özelliklerini değerlendirmek amacıyla düzenlendi. Çalışma grubumuza katılan hastaların %87.3’ünü erkekler oluşturdu. Grubun %76.1’i bekardı. Ortalama hastalık başlangıç yaşı 15.53±11.93 olarak bulundu. Hastaların %20.5’i üniversite mezunuydu. %50.2’sinin hiçbir sosyal güvencesi yoktu. Hastaların eğitim düzeyi ile hastalık hakkında bilgi düzeyi, son bir yıldaki nöbet sıklığı, ilacı düzenli kullanması, hastalıkla ilgili kontrole gitmesi ilişkili bulundu. Çalışmamızda epilepsili hastaların epilepsi hakkında yeterli bilgiye sahip olmadığı ve bazı hastalarda hastalıkla ilgili mistik inançların olduğu belirlendi. Hastaların antiepileptik ilaç yan etkilerine ileri derecede maruz kaldıkları, hastaların büyük çoğunluğunda iyi bir nöbet kontrolü sağlanamadığı anlaşıldı. Sonuçlar, hasta ve yakınlarının bilgilendirilmesi, mistik inançları yok etmek için epilepsinin topluma daha iyi tanıtılması, epilepsili bireylere özellikle hemşirelerin aktif rol aldığı multidisipliner ekip yaklaşımı ile hizmet verilmesi hastaların tedaviye uyumunu artıracağını göstermektedir.
EVALUATION OF PATIENTS WITH EPILEPSY CONFORM TO ANTIEPILEPTIC MEDICATION This study has been done to evaluate patients’ knowledge and understanding about their epilepsy, to asses which factors affect to conform to medical care, to put forward proposals which will guide in the planning of health care neccessities and to emphasize necessity of epilepsy nursing for our country. This study was realized with 205 patients with epilepsy who applied to neurology policlinics, Gülhane Military Medicine Academy Haydarpaşa Hospital beetween the dates of 01.09.2005-01.03.2006. Patients with epilepsy who have been at the age of 18 and over, have accepted to answer the questions, have read and written Turkısh, have taken antiepileptic drugs for at least two year, have not had psychiatric problem were included this study. According to the literature on this field the Patient Interwiew Questionnaire has been used as a means of collecting data. The Patient Interwiev Questionnaire has been developed to evaluate level of knowledge about epilepsy, patients’ personal, socio-demographic and illness characteristic. %87.3 of patients who have participated to this study is male. %76.1 of the participants are single. Mean age at onset of epilepsy in the study population is found 15.53±11.93. %20.5 of patients are graduated from university. %50.2 of patients have no health insurance. It is found that education level is related with knowledge about illness, duration of seizure, using medicine regularly, going to control about illness. In our study, ıt is found that patients with epilepsy have not have enough information about epilepsy and some of them have had mystical beliefs related to illness. It is realized that patients have been over exposed to the side effects antiepileptic drugs and have not well controlled seizure. As a result of this study, patients’ conforming to the medical care would increase If the patient and family are informed to control the disease, the epilepsy is presented better to the puplic in order to prevent mystical beliefs and they are served with multidiciplinary team aproach which nurses take active role.
