Publication: Türkiye’de sağlık sektörünün değişim dinamikleri ve değişen çalışma ilişkileri
Abstract
Günümüz küresel dünyasında özellikle 1980 sonrası dönemde yeni liberal politikaların neredeyse tüm devletlerce hâkim kılınan ekonomi politikası haline gelmesiyle karşılıklı ilişki içerisinde olduğu pek çok sektöre de etkileri olmuştur. Ülkemizde 24 Ocak kararlarıyla birlikte Yeni liberal politikaların uygulanmaya başlandığı dönem itibariyle söz konusu bu politikaların sağlık sektörüne olan yansımaları gecikmemiş, kendini “Sağlık Reformu” ve/ veya “Sağlıkta Dönüşüm” başlıkları altında bulmuştur. Böylece sağlık hizmetlerinin gerek sunumu gerekse finansmanında değişimler meydana gelmiş, devletin sağlık hizmetlerine yaklaşımında dönüşüm sürecine girilmiştir. Uluslar arası aktörlerin yapısal uyum programları çerçevesinde dayattığı birtakım uygulamalarla dönüşüm süreci hız kazanmış, bu kurum ve kuruluşlar değişim dinamiklerinin temelini oluşturmuştur. Sağlık adeta parası olanın parası kadar satın alabildiği bir meta haline dönüştürülmüş, bu çerçevede sağlık hizmetlerinin piyasaya devri, sağlık insan gücünün “esnekleştirilmesi” vb. uygulamalar, sağlığı piyasa koşullarına bırakan gelişmeler olarak izlenmiştir. Esnekleşme olgusu beraberinde, gerek ücretler ve istihdam yönünden, gerekse iş güvencesinden yoksun bir işgücü ordusuyla karşı karşıya kalınması sonucunu getirmiştir yeni liberal politikalar, sağlık sektörü, değişim dinamikleri, esneklik
In today’s global world, especially in the period after 1980 almost all states by dominant neo- liberal policies of the economic policy making in becoming the mutual relations that have been influenced in many sectors. In our country, with the January 24 decision of the neo- liberal policies started to be implemented as of the periods in question these policies to the health sector which has not delayed reflections, self “Health Reform” and “Health Transition” was found under headings. Thus a need for health care delivery changes occurred both in the financing, the state’s approach to health services was entered in the transformation process. International actors in the framework of strutural adjustment programs imposed some practices that have accelerated the process of trnasformation, these institutions and organizations have formed the basis for dynamic change. Health benefit is almost a commodity can be purchased from the Money up, converted into, in this context, transfer of health care markets, health manpower “flexible upgrading”, ect.. practices, health conditions have left the market developments are monitored. Case along with a flexible, both in terms of wages and employment, as well as a lack of job security to be confronted with a labor army brought results.
