Cephalometric evaluation of class II patients treated with keles slider

Abstract

Bu çalışmanın amacı, sınıf II hastalarda devrilme olmadan ve en az ankraj kaybıyla molarların gövdesel distal hareketini sağlayacak Keles Slider'ın etkilerini görmekti. Bilateral molar distalizasyonu için 10 hasta (7 kız, 3 erkek) seçildi, ortalama yaş 15,9'du. Tüm hastalarda her iki tarafta da dişsel sınıf II molar ilişkisi mevcut olup, alt diş arkı düzgündü ve normal veya sagital yönlü büyüme paterni sergilemekteydi. Aktif distalizasyon periyodundan sonra alınan model fotkopileri ve lateral sefalometrik filmlerin analizinde 37 parametreden 17'sinin belirgin değişimler gösterdiği saptandı. Bu değişimler şöyle sıralanıyordu: üst 1. molarlar hiç devrilme olmadan ortalama 4.00 mm distale hareket etmişler 1.25 derece distobukkal rotasyon yapmışlar ve intermolar genişlik 2 mm artmıştı.. Üst 1. premolarlar ortalama 2.70 mm mesiale hareket etmişler, 1.9 derece devrilmişler ve 0.50 mm ekstrüze olmuşlardı. Üst santral kesiciler 2.05 mm prokline olmuşlar ve 3.45 derece devrilmişlerdi. Alt 1. molarlar ortalama 0.75 mm ekstrüze olmuşlar, overjet 2.05 mm artmış ve overbite 2.05 mm düşmüştü. Retansiyon döneminden sonra lateral sefalometrik filmlerin analizindeki belirgin değişimler ise şöyleydi: üst santral keserler 0.65 mm mesial drift yapmışlar ve 1.15 derece linguale devrilmişlerdi. Üst 1. premolarların mesial migrasyonu 1.30 mm azalmıştı. Overjet 0.75 mm azalmış ve overbite 0.75 mm artmıştı. Yukarıdaki sonuçlar bize minimum ankraj kaybıyla ve hasta kooperasyonuna gerek duyulmadan Keles Slider'in gövdesel distal molar hareketi yaptırdığını göstermiştir. The aim in this study was to see the effects of keles slider in class II patients where we can achieve bodily distal movement of molars without tipping and with least anchorage loss. Ten patients, seven females and three males with a mean age of 15.9 years, were selected for bilateral molar distalization. All patients had dental class II molar relationship on both sides, lower arch was well aligned and showed normal or sagitally directed growth pattern. Analysis of lateral cephalometric films and model photocopies after active period of distalization showed that 17 parameters out of 37 parameters denoted significant changes. These important changes were as follow: upper first molars were moved distally by 4mm without any tipping, rotated distobuccaly by 1.25 degree and the intermolar width increased by 2mm. Upper first premolars were moved mesially by 2.70mm, tipped by 1.9 degree and extruded by 0.50mm. The upper central incisors were proclined by 2.05mm and tipped 3.45. The lower first molars were extruded by 0.75mm.the over jet was increased by 2.05mm and the over bite was decreased by 2.05mm. Analysis of the lateral cephalometric films after retention period showed that Upper central incisors drifted mesially by 0.65mm and tipped 1.15 degree lingually. Mesial migration of upper first premolars reduced by 1.30mm.The over jet was reduced by 0.75mm and over bite increased by 0.75mm. The above results show that Keles slider achieved bodily distal molar movement with minimum anchorage loss and without patient cooperation.
