Koroner yoğun bakım ünitesine başvuran hastaların uyku sorunları ve uyku sorunlarını etkileyen faktörlerin değerlendirilmesi

Abstract

KORONER YOĞUN BAKIM ÜNİTESİ'NE BAŞVURAN HASTALARIN UYKU SORUNLARI VE UYKU SORUNLARINI ETKİLEYEN FAKTÖRLERİN DEĞERLENDİRİLMESİ Adı ve Soyadı : Emine İNCEKARA Danışmanın Adı: Rukiye PINAR Program : Yüksek Lisans Hastalıkları Bu çalışma Koroner Yoğun Bakım Ünitesinde (KYBÜ) yatan 160 hastanın uyku sorunu yaşayıp yaşamadığı ve bu sorunun özellikle hastane anksiyete ve depresyonu ile ilişkili olup olmadığını belirlemek amacıyla yapılmıştır. Veriler genel bilgi formu, Pittsburg Uyku Kalite İndeksi (PUKİ) ve Hastane Anksiyete ve Depresyon Ölçeği ile toplanmış; ki-kare, student t testi, lojistik regresyon analizi ve Pearson's korelasyon analizleri ile değerlendirilmiştir. We found that hastaların yarıdan fazlası uyku sorunu yaşamaktadır (54.4%). PUKİ kullanılarak yapılan değerlendirmede hastaların hastalık öncesi dönemde %4.4'ünün uyku sorunu yaşarken, bu oranın son bir hafta içinde %3.1'lik artışla %7.5'e çıktığı belirlenmiştir. Uyku sorununa yol açan üç temel neden hastalıkla ilgili kaygı (%59.8), ağrı (%54.0) ve gürültüdür (%37.9). Birden fazla nedenle uyku sorunu yaşayan olguların oranı %33.3'tür. Erkekler kadınlara göre 0.21 kat (P<0.01), üç neden birarada olanlar 0.17 kat (P<0.05) daha fazla uyku sorunu yaşamaktadır. Olguların %53.1'i depresyon, %20.6'sı anksiyete yaşamakta, %19.4 olguda ise anksiyete ve depresyon birlikte deneyimlenmektedir. Uyku sorunu anksiyete ve depresyondan bağımsızdır. Sonuç olarak, KYBÜ'nde yatan hastaların uykularının düzenli aralarla değerlendirilmesi, hastalıkla ilgili kaygının önlenmesi ve giderilmesi için liyezon psikiyatrisi ile işbirliği yapılması, ağrının, uyku sorununa yol açan çevresel faktörlerin ve özellikle gürültünün kontrol altına alınması önerilmiştir. EVALUATION of the SLEEP DISTURBANCES and AFFECTING FACTORS in PATIENTS WHO ADMITTED to the CORONARY INTENSİVE CARE UNIT The study has been performed to identify whether 160 patients hospitalized in Coroner Intensive Care Unit (CICU) experience sleep disturbances and if these problems are related with hospital anxiety or depression. The data of the research were gathered via the general information form, Pittsburgh Sleep Quality Index (PSQI) and Hospital Anxiety and Depression Scale and were evaluated using chi-square, students t test, logistic regression analysis and Pearson's correlation analysis. We found that more than half of the patients experienced sleep disturbances (54.4%). The assessment performed with PSQI showed that 4.4% of the patients experienced sleep disturbances before hospitalization, while the percentage increased with a 3.1% to 7.5% within the last week. Three major causes of sleep disturbances were the anxiety related with the sickness (59.8%), pain (54.0), and noise (37.9%). The rate of the subjects experiencing sleep disturbances due to more than one of the causes was 33.3%. Males had 0.21 times more sleep disturbances than females (P<0.01), and subjects with multiple causes experienced sleep disturbances 0.17 times more than others (P<0.05). Of the subjects, 53.1% experienced depression, and 20.6% anxiety, while 19.4% of the subjects experienced depression and anxiety simultaneously. Sleep disturbances are independent from anxiety and depression. In conclusion, it is suggested that the sleep patterns of the patients hospitalized at CICU be periodically observed, that a cooperation be established with liaison psychiatry for the prevention and elimination of the anxiety stemming from the sickness, and that pain, environmental factors that lead to sleep disturbances - particularly noise- be controlled.
