Meme operasyonu sonrası gelişen üst ekstremite lenfödeminde düşük doz lazer tedavisinin etkinliği

Abstract

Meme Operasyonu Sonrası Gelişen Üst Ekstremite Lenfödeminde Düşük Doz Lazer Tedavisinin Etkinliği Çalışmamızın amacı meme operasyonu sonrası lenfödem gelişen hastalarda düşük doz lazer uygulamasının etkilerini incelemektir. Bu kapsamda çalışmaya mastektomi sonrası unilateral lenfödem gelişen 33 kadın hasta dâhil edildi. Olguların kollarının çevre ölçümleri alınarak volümetrik değerleri hesaplandı. Ayrıca el kas gücü ölçümü ve ağrı değerlendirmeleri de yapıldı. Katılımcılar basit eşleştirme yöntemiyle tedavi ve kontrol grubu olarak 2‘e ayrılarak tedavi gubuna pnömatik kompresyon ve düşük doz lazer uygulaması diğer gruba pnömatik kompresyon ve plasebo lazer uygulaması yapıldı. Tedavi üç hafta boyunca haftada beş gün olacak şekilde planlandı ve düşük doz lazer tedavisi birer gün arayla uygulandı. Olgular tedavi öncesinde, tedavi bitiminde ve bitiminden 1 ay sonra değerlendirildi. Değerlendirmelerin sonunda lenfödemli kol hacminde hem kontrol hem de tedavi grubunda anlamlı azalma tespit edildi ancak iki grup arasında anlamlı fark bulunamadı (p>0,05). Kas gücündeki değerlendirmemizde her iki grupta anlamlı iyileşme görüldü (p<0,05), tedavi sonrasında gruplar arasında anlamlı fark bulunurken (p<0,05), tedavi sonrası 1.ayda anlamlı fark bulunamadı (p>0,05). Başlangıca göre ağrı değerinde her iki grupta anlamlı düzelme görüldü (p<0,05), tedavi sonu ve 1. ay kontrollerde de iki grup arasında anlamlı fark olduğu tespit edildi. Sonuç: Mastektomi sonrası gelişen lenfödem de pnömatik kompresyon uygulamasına ek olarak yapılan düşük doz lazer tedavisinin klinik olarak yararlı olmadığı görüldü. Anahtar Sözcükler: lenfödem, düşük doz lazer, pnömatik kompresyon, mastektomi EFFICIENCY of LOW-LEVEL LASER THERAPY UPON POSTMASTECTOMY LYMPHEDEMA of UPPER LIMBS The aim of our study is to investigate the effects of low level laser therapy among patients with postmastectomy lymphedema. Thirtythree women having postmastectomy unilateral lymphedema were included. Volumetric values were calculated by measuring arm rounds of the cases. Hand muscles strength and pain evaluations were also done. Cases divided into two groups by simple matching method. Pneumatic compression and low-level laser therapy were applied to the first group and pneumatic compression and placebo laser were applied to the other group. Each groups were treated 5 days per week for three weeks, one day apart, were administered a low level laser. Cases were evaluated before, at the end of and at the first month of the treatment. A significant reduction in volumes of arms suffering from lymphedema was revealed in both groups. No significant difference was revealed between two groups (p>0,05). There was a significant improvement of muscle strength in each group (p<0,05), the groups were significantly different at the end of the treatment (p<0,05), nevermore there was no significant difference at the end of the first month after treatment between the groups. Pain levels of both two groups were relieved significantly (p<0,05).There was also significant difference in controls at the end of the treatment and the first month of the treatment. Conclusion: It is revealed that low-level laser therapy beside pneumatic compression was not clinically efficacous among the patients with postmastectomy lymphedema.
