Chiari Malformasyonları

Abstract

Chiari malformasyonları (CM), serebellar tonsillerin foramen magnum’dan basit şekilde herniasyonundan, serebellar ageneziyeuzanan geniş bir yelpazedeki arka beyin anomalilerini kapsar. Tüm CM tipleri arka beyin yapıları ile ilişkili olsa da tiplerin etiyolojisive patofizyolojisi birbirinden farklılık gösterir. Semptomatik hastalarda tedavinin amacı, posterior fossa dekompresyonu ilekranyovertebral bileşke etrafındaki bozulmuş olan beyin omurilik sıvısı (BOS) dinamiklerini tekrar düzenlemektir. Klinik prezentasyon,görüntüleme bulguları ve cerrahi dekompresyon tekniği her CM tipi için farklılık arz eder. Çalışmadaki amacımız, CM terminolojisinigözden geçirmek ve her bir tipi patofizyoloji, klinik prezentasyon, tanı ve görüntüleme yöntemleri, tedavi stratejileri ve komplikasyonlaraçısından detaylı bir şekilde ele almaktır.

The Chiari malformations (CMs) are a spectrum of hindbrain abnormalities ranging from simple tonsillar herniation through the foramen magnum to complete cerebellar agenesis. Even if all the CM types are associated with the hindbrain structures, their etiology and pathophysiology are different from each other. The goal of the treatment in symptomatic patients is to restore the normal cerebrospinal fluid (CSF) dynamics across the craniovertebral junction by decompressing the posterior fossa. The clinical presentation, imaging findings, and surgical techniques of decompression vary widely for each type of CM. In this study, we aim to review the terminology of CM, as well as the pathophysiology, clinical presentations, diagnostic methods, treatment strategies, and complications for each CM type.
