Publication: Çeşitli kanserli dokularda tromboplastik aktivite ve protein dağılımlarının poliakrilamid jel elektroforezi ile incelenmesi
Abstract
Tromboz ve kanser arasında bir ilişki olduğu uzun zamandan beri bilinmesine rağmen, bu ilişki hakkında bilgilerimiz günümüzde hala netleşmemiştir, hemostazın bu konudaki önemi tartışılmakta ve doku faktörünün kanser gelişiminde de önemli olduğu bildirilmektedir. Bu çalışmanın amacı; kanser tutulumu olan ve olmayan çeşitli dokularda tromboplastik aktiviteyi incelemek, bu dokuların protein dağılımlarını SDS-PAGE ile görmek ve aynı hastaların kanlarında protrombin zamanı, faktör VIIa, fibrinojen ve APTT gibi hemostatik parametrelerin değişimlerini görmektir. Çalışmada Şişli Etfal Eğitim ve Araştırma Hastanesinde cerrahi rezeksiyonla alınıp patoloji kliniğine gönderilmiş olan çeşitli kanserli dokular kullanıldı. Patoloji kliniğinde ayırımı yapılan dokuların homojenizasyonu yapıldı. Çalışma grubumuzu oluşturan toplam 37 hastadan eşleştirebildiğimiz 31 çeşitli kanserli doku ile aynı dokuların kanser tutulumu olmayan bölgelerine ait tromboplastik aktiviteleri arasında ileri derecede anlamlı fark vardır (p=0.003). Bazı koagulasyon parametreleri ile mide, kolon, meme kanseri arasında ilişkili olduğunu gördük. Bu korelasyonlar ve anlamlılıkları sırasıyla şöyledir: r=0,980, r=0,985 (p<0,05); r=0,952, r=-0,837, r=0,825, r=987 (p<0,01- p<0,05); r= -1,000, r=1,000, r=1,000 ( p<0,01). Kanserli dokuların bazılarında tromboplastik aktivite artmıştır. Aynı hastaların hemostatik parametrelerinde benzer bir anlamlılık tespit edilememiştir. Yani kimi kanserli olgularda görülen dokudaki tromboplastik aktivite kana benzer şekilde yansımamıştır. Dokudaki tromboplastik aktivite artışı yaş ve cinsle ilgili bir değişim de göstermemiştir.Malign hücreler çeşitli oranlarda doku faktörü ekspresyonu yapabilmektedir. Kanserli dokular tromboplastik aktivite yönünde burada gösterdiğimiz gibi Quick yöntemi ile değerlendirilip prognoz hakkında bilgi edinilerek tedavi yönlendirilebilir. An association between cancer and thrombosis has been known for many years. However, the manner by which tumor growth is regulated by coagulation in vivo remains unclear. It has been shown that a functional role of hemostasis in early cancer development and tissue factor expression plays potential role in cancer metastasis. The aim of the present study, was to elucidate tissue factor activity and protein distribution in different types of tumor cells and normal cells of the same tissue. Some blood coagulation parameters were also investigated in plasma such as factor VIIa, protrombin time, APTT and fibrinogen. Tissue factor homogenates were prepared from 37 tissues with different cancer types and 31 of them also had normal tissues of the same cancer patients. The tissues were extracted from the patients that had been hospitalized in Şişli Etfal Education and Research Hospital. After surgical operation diagnosis was performed on tumor resections in pathology department. According to our results tissue factor activity which was analysed using the tissue exctracts of the cancer patients showed dramatic increase in some of the carcinoma compared to their noncarcinomae areas (p=0.003). Some correlations were obtained between coagulation parameters and stomach, colon and breast cancer tissue factor activities.The results being r=0,980, r=0,985 (p<0,05); r=0,952, r= -0,837, r=0,825, r=987 (p<0,01- p<0,05); r= -1,000, r=1,000, r=1,000 ( p<0,01) respectively. In the present study tissue factor initiated by thrombin generation was increased in some of cancers tissues compared to their control tissues. Malignant cells from patients with different cancers produce various levels of tissue factor. Some cancers are more likely to be prothrombotic than others. Our study was performed according to Quick method. It is a simple and suitable test.
