Publication: Aripiprazol ve paliperidon içeren farklı farmasötik formları kronik kullanan şizofreni hastalarında etkinlik ve güvenliliklerinin terapötik düzeyleriyle değerlendirilmesi
Abstract
Amaç: Bu çalışmada, kronik uzun etkili atipik antipsikotikleri şizofreninin idame tedavisinde kullanmakta olan hastalarda ilaç konsantrasyonlarındaki değişiklikler, etkililik, advers etkiler ve güvenlilik arasındaki ilişkilerin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Tedavilerine 4 aydan uzun süredir aripiprazol 1-Aylık 400 mg, paliperidon palmitat 1-Aylık 100 mg veya Paliperidon 3-Aylık 350 mg preparatlarla devam etmekte olan şizofreni hastalarının 4 ay boyunca ilaç düzey takipleri yapılmıştır. Semptomlar ve advers etkiler psikometrik ölçeklerle, ilaç düzeyleri ise LC-MS ile değerlendirilmiştir. Bulgular: PANSS negatif ve toplam skorların aripiprazol 1-aylık grubunda paliperidonun 3-aylık formuna göre skorların istatistiksel olarak daha düşük olduğu izlenmiştir (p<0,05). İstenmeyen cinsel etkilerin aripiprazol grubunda istatistiksel olarak belirgin düzeyde diğer gruplara göre düşük olduğu izlenmiştir (p<0,001). Odaklanma ve bellek güçlüklerini deneyimleyen kişi sayısı paliperidon 3-aylık grubunda istatistiksel olarak paliperidon 1- aylık grubuna göre daha az bulunmuştur (p<0,05). İlaç düzeyleri karşılaştırıldığında aylar arasında istatistiksel olarak anlamlı farklılık göstermeyen stabil bir seyir izlenmiştir (p>0,05). Sonuç: İdame tedavide aripiprazolün paliperidon preparatlarına kıyasla daha etkili ve güvenli olduğu izlenmiştir.
Aim: This study, aimed to assess the relationships between changes in blood drug concentrations, efficacy, adverse effects and safety in patients using chronic long-acting atypical antipsychotics in the maintenance treatment of schizophrenia. Materials and Methods: Blood levels of patients with schizophrenia who were on maintenance therapy and were using Aripiprazole 1-Month 400mg, Paliperidone Palmitate 1-Monthly 100 mg and Paliperidone 3-Month 350 mg preparations for more than 4 months were followed for 4 months. Symptoms and adverse effects were evaluated with relevant psychometric scales and drug levels with the LC-MS method, Results: It was observed that PANSS negative and total scores were statistically lower in the 1-month-old aripiprazole group than in the 3-month-old form of paliperidone (p<0.05). The evaluation of sexual adverse effects revealed that it was statistically significantly lower in the aripiprazole group compared to the other groups (p<0.001). The number of people experiencing focusing and memory difficulties was statistically lower in the paliperidone 3-month group than in the paliperidone 1-month group (p<0.05). A stable course was observed in drug levels within the groups (p>0.05). Conclusion: It has been observed that aripiprazole preparation is more effective and safe compared to paliperidone preparations in maintenance therapy.
