Publication: Normal işiten infant ve çocuklarda auditory steady state response ve tonal işitsel beyinsapı cevap odyometrisinin karşılaştırılması
Abstract
1. ÖZET Bu çalışmada, normal işiten infant ve çocuklarda, Auditory Steady State Response (ASSR) ve tone burst işitsel beyinsapı cevap odyometrisi (T-İBC) bulgularının değerlendirilmesi ve T-İBC’de V. dalga latans değerlerinin farklı yaş gruplarında karşılaştırılması amaçlanmıştır. Normal işiten 74 infant ve çocuk çalışma kapsamına alınmıştır. Konsepsiyonel yaşı 40 hafta-1.5 yaş arasında olan infant ve çocuklar değerlendirilmiştir. T-İBC latans değerlendirmeleri ise 4 farklı grupta yapılmıştır. İlk önce infant ve çocukların işitmeleri Transient Evoked Otoakustik Emisyon (TEOAE), Akustik Immitansmetri ve klik İBC ile değerlendirilmiş, daha sonra tone burst işitsel beyinsapı cevap odyometrisi ve ASSR uygulanmıştır. T-İBC ve ASSR testlerinin karşılaştırılmasında, önce Mc Nemar testi ve paired t-testi kullanılmıştır. Daha sonra Bland ve Altman yöntemi ve ‘’ uyum limitleri’’ çizimi kullanılmıştır. Yaş grupları arasındaki ortalama latans farklılıkları için tek yönlü ANOVA ve Tukey çoklu karşılaştırma testleri yapılmıştır. Normal işiten infant ve çocuklarda, ASSR tek frekans uygulaması ve T-İBC arasında uyum olduğu saptanmıştır. Bu sonuca göre ASSR işitmenin değerlendirilmesinde yardımcı bir test yöntemi olarak kullanılabilir. T-İBC latans değerlendirmesi sonucu, 500 Hz ve 4000 Hz’de 0-2 aylık grup ile diğer gruplar arasında elde edilen farklar, maturasyonun bu grupta belirgin olarak tamamlanmadığını göstermektedir. Comparison of Auditory Steady State Response and tone burst Auditory Brainstem Response among normal hearing infant and children. In order to compare T-ABR and ASSR tests, initially, Mc Nemar and paired t-test were utilized. And then, together with Bland and Altman method, ‘limit of agreement’ drawing was utilized. Single ANOVA and Tukey-multiple comparison tests were realized for average latency difference among age groups. It was found that there is a relation between ASSR single frequency application and T-ABR among normal hearing infants and children. According to this result, ASSR can be utilized as a supporting test method for hearing measurement. According to the T-ABR latency evaluation result, i.e. the difference between 0-2 month’s infant group and other groups at 500 Hz and 4000 Hz, maturation was not completed for 0-2 month’s infant group.
