Hormon replasman tedavisinin gözyaşı fonksiyonları ve kornea hassasiyeti üzerine etkisi

Abstract

Amaç: Kuru göz, menopoz sonrası kadınlarda sıklıkla görülen ve aynı yaş grubu erkeklere oranla kadınların daha sık olarak etkilendiği bir hastalıktır. Bunun muhtemel bir sebebi, menopozda azalan doğal östrojen miktarıdır. Hormon replasman tedavisinin (HRT) sistemik etkileri (vazomotor semptomlar, kalp hastalıkları vb.) hakkında pek çok veri olmasına rağmen, HRT' nin ve menopozun kuru göz ile bağlantısı hakkında az sayıda çalışma mevcuttur. Bu çalışmada, hormon replasman tedavisinin kuru göz semptomları, gözyaşı fonksiyon testleri ve kornea has-sasiyeti üzerine olan etkisini araştırmayı amaçladık. Yöntem: Çalışmaya yaşları ve amenore süreleri uyumlu, HRT alan 30 ve almayan 32 me-nopoz sonrası kadın dahil edildi. İki grubun gözyaşı fonksiyon testleri (Schirmer I testi, anestetikli Schirmer testi, Rose Bengal boyanma skoru ve gözyaşı kırılma zamanı), kuru göz semptom skorları ve kornea hassasiyetleri saptanarak birbirleriyle karşılaştırıldı. Bulgular: HRT alan ve almayan olguların kuru göz semptom skorları, (1.3 ± 1.4; 1.7±1.8) Schirmer I testleri, (17.5 ± 7.8; 18.4 ± 8.6 mm/ 5 dak) anestetikli Schirmer testi ölçümleri, (9.7 ± 5.5; 10.4 $\pm$6.1 mm/5 dak) gözyaşı kırılma zamanları, (11.6 ± 4.1; 10.3 ± 6 sn) Rose Bengal boyanma skorları, (0.3 $\pm$ 0.7; 0.6±0.8) esteziyometre ölçümleri, (5.3 ± 0.5; 5.5±0.5 mm/g) olarak bulundu ve iki grup arasında istatiksel olarak anlamlı bir farklılık saptanmadı (p>0.05). Tartışma: Bu çalışmada HRT' nin kuru göz semptomları, gözyaşı fonksiyon testleri ve kornea hassasiyeti üzerine menopoz sonrası dönemde olumlu bir etkisi gösterilememiştir.

Aim: Dry eye is a common disease in postmenopausal women, affecting them significantly more often than men of the same age group and one possible reason for its occurrence is the reduction in naturally occurring estrogen during menopause. Although we know much about the systemic effects (vasomotor symptoms, cardiovascular effects etc.) of hormone replacement therapy (HRT), very few studies are present about the relations between HRT and dry eye. The aim of this study is; to evaluate the influence of hormone replacement therapy on dry eye symptoms, lacrimal tests and corneal sensitivity.Methods: Sixty-two (matched for age and duration of amenorrhea) postmenopausal women; 30 receiving and 32 not receiving HRT were enrolled in this study. Subjects underwent a series of diagnostic tests that included Schirmer 1 test, Schirmer test with anesthesia, tear break-up time and rose bengal staining of the ocular surface and corneal sensitivity measurement. Results: No statistically significant differences were found between patients receiving or not receiving HRT regarding the dry eye symptom scores, (1.3 ±1.4; 1.7$\pm$1.8), Schirmer I tests, (17.5 ± 7.8; 18.4 $\pm$ 8.6 mm/ 5 min), Schirmer test with anesthesia, (9.7 ± 5.5; 10.4 ±6.1 mm/5 min), tear break up time, (11.6 ± 4.1; 10.3 $\pm$6 sec), rose bengal scores, (0.3 ± 0.7; 0.6±0.8) and esthesiometer measurements, (5.3 $\pm$ 0.5; 5.5$\pm$0.5 mm/g). Discussion: In this study we could not demonstrate any positive effect of hormone replacement therapy on dry eye symptoms, lacrimal function tests and corneal sensitivity.
