Serbest eczanede klinik eczacı tarafından hipertansif hastalara verilen farmasötik bakım hizmetlerinin sonuçlarının değerlendirilmesi

Tezin Başlığı: Serbest Eczanede Klinik Eczacı Tarafından Hipertansif Hastalara Verilen Farmasötik Bakım Hizmetlerinin Sonuçlarının Değerlendirilmesi Öğrencinin Adı, Soyadı: Emine Ceren Demircioğlu Akyılmaz Amaç: Klinik eczacı tarafından serbest eczanede verilecek farmasötik bakım hizmetlerinin hipertansif hastalarda ilaca uyum öz-etkililiğine katkısının ölçülmesi, güncel hipertansiyon kılavuzlarında hedeflenen kan basıncı değerlerine ulaşılmasındaki etkisinin incelenmesi ve 40 yaş üstü, hipertansif tanısı almamış bireyleri erken tanı için doktora yönlendirmeyi içeren klinik eczacılık danışmanlığının etkisinin değerlendirilmesi amaçlanmıştır. Gereç ve yöntem: Çalışmaya dahil edilen hastalar farmasötik bakım grubu ve kontrol grubu şeklinde ikiye ayrılmıştır. Her iki grubun kan basıncı değerleri periyodik olarak kayıt altına alınmış, hastalara çalışma başlangıcında ve sonunda olmak üzere ‘Hipertansif Hastalarda İlaca Uyum Öz-Etkililik Ölçeği Kısa Formu’ uygulanmıştır. Çalışmanın ikinci bölümü hipertansiyon tanısı almamış 40 yaş üstü bireylerle gerçekleştirilmiştir.Yüksek kan basıncına sahip kişiler doğrudan doktora yönlendirilmiş ve tanı alıp almadıkları izlenmiştir. Bulgular: Çalışmanın ilk bölümüne hipertansiyon tanısı konmuş olan 81 kişi dahil edilmiştir. Çalışmanın sonunda farmasötik bakım grubundaki hastaların sistolik kan basıncı medyan değeri 150 mmHg’dan (ÇAA:137,2-160) 135 mmHg’ya (ÇAA:130-143) düşmüştür. Farmasötik bakım grubunun sistolik kan basıncında kaydedilen bu fark kontrol grubuna göre istatistiksel olarak anlamlıdır (p<0,05). İlaca uyum öz-etkililik ölçeğinde toplam skordaki artış kontrol grubuna kıyasla istatistiksel olarak anlamlıdır (p<0,001). Çalışmanın diğer bölümüne 62 kişi dahil edilmiş, riskli bulunduğu için doktora yönlendirilen 23 kişiden 12’sine (%52,2) doktor tarafından hipertansiyon tanısı konulduğu ve 8 kişinin (%34,7) prehipertansif olduğu belirlenmiştir. Sonuçlar: Çalışma sonuçlarına göre klinik eczacı tarafından sunulan farmasötik bakım programının hipertansiyon tedavisi yönetimine, hasta uyumuna, tarama ve erken tanıya yönlendirmeye olumlu etkisi olduğu gösterilmiştir.

1.SUMMARY Title of the Thesis: Evaluation of Pharmaceutical Care Services Provided to Hypertensive Patients by the Clinical Pharmacist at the Community Pharmacy Name of Student: Emine Ceren Demircioğlu Akyılmaz Objectives: The purpose of this study is to evaluate the contribution of the pharmaceutical care services provided by the clinical pharmacist at the community pharmacy according to the self-efficacy of adherence to medication in hypertensive patients and to determine impact of clinical pharmacist-led service among individuals over the age of 40 without diagnosis of hypertension. Method: The patients were divided into pharmaceutical care group and control group. The blood pressure values of both groups were recorded periodically, and the 'Short Form of Medication Adherence Self-Efficacy Scale in Hypertensive Patients' was applied at the beginning and end of the study. The second part of the study was conducted with individuals over the age of 40 who had not yet been diagnosed with hypertension. People with high blood pressure were referred directly to the physician, and their status of hypertension was monitored. Results: In the first part of the study, 81 patients diagnosed with hypertension were included At the end of the study, the median systolic blood pressure of patients in the pharmaceutical care group decreased from 150 mmHg (IQR: 137,2-160) to 135 mmHg (IQR: 130-143). This difference recorded in the systolic blood pressure of the pharmaceutical care group was statistically significant compared to the control group (p<0,05). The increase in the total score in the adherence self-efficacy scale was statistically significant compared to the control group (p<0,001). In the other part of the study 62 individuals were included, it was determined that 12 (%52,2) of 23 people who were referred to the doctor because they were found to be risky were diagnosed with hypertension by the medical doctor and 8 people (%34,7) were prehypertensive. Conclusions: According to the results of the study, it has been shown that the clinical pharmacist-led pharmaceutical care program has a positive effect on the management of hypertension treatment, patient compliance, screening and early diagnosis.
