Hemodiyaliz hastalarına verilen eğitimin beslenme yönetimine etkisi

Abstract

1.ÖZET Tez Adı: Hemodiyaliz Hastalarına Verilen Eğitimin Beslenme Yönetimine Etkisi Öğrenci: Özlem Bulantekin Düzalan Danışman: Doç. Dr. Sezgi Çınar Pakyüz İç Hastalıkları Hemşireliği alışmanın amacı; hemodiyaliz hastalarına hemşire tarafından verilen eğitimin beslenme yönetimine etkisini değerlendirmektir. Metodolojik ve deneysel olarak yapılan bu çalışma iki aşamada gerçekleştirildi. İlk aşamada; bu araştırma için 19 maddelik 3’lü likert tipte Hemodiyaliz Hastalarının Diyet Bilgi Ölçeği (HHDBÖ) ve 18 maddelik 5’li likert tipte Hemodiyaliz Hastalarının Diyet Davranış Ölçeği (HHDDÖ) araştırmacı tarafından geliştirildi. HHDBÖ ve HHDDÖ için Cronbach alfa güvenirlik katsayısı sırasıyla 0,86, ve 0,73’tür. İkinci aşamada; 80 hemodiyaliz hastası kontrol ve deney grubuna ayrıldı, deney grubu hastalarına hemşire araştırmacı tarafından diyet ve sıvı kontrolü hakkında eğitim verildi. Verilen eğitim sonrasında; HHDBÖ ve HHDDÖ son test puanlarının ön test puanlarına göre anlamlı derecede arttığı (p<0,05), serum sodyum miktarının, iki diyaliz arası kilonun ve diyastolik kan basıncının istatistiksel olarak anlamlı derecede azaldığı (p<0,05) belirlendi. Deney grubunda hemodiyaliz yaşı arttıkça ön test HHDBÖ ve HHDDÖ puanları arasında pozitif yönde korelasyon olduğu belirlendi (p<0,05). Kontrol grubunda ise HHDBÖ ve HHDDÖ son test puanlarının ön test puanlarına göre anlamlı derecede düştüğü (p<0,05) belirlendi. Sonuç olarak; hemodiyaliz hastalarına hemşire tarafından verilen eğitimin beslenme yönetimine olumlu yönde etkisi olduğu belirlendi. Hemodiyaliz hastalarının diyet ve sıvı kısıtlaması hakkındaki bilgilerini artırmak ve olumlu davranış değişikliği yaratmak için hemşire tarafından düzenli eğitim verilmesi önerilmektedir. Thesis Name: The Effects Of Given Education To Hemodialysis Patients On Nutritional Management Student: Özlem Bulantekin Düzalan Supervisor: Assistant Professor Doctor Sezgi Çınar Pakyüz Department of Internal Diseases Nursing The aim of this study was to evaluate the effect of training in nutrition management that is given to the haemodialysis patients by nurses. This study that was designed as methodological and experimental was carried out in two stages. In the first stage; the researcher developed an Information Scale with 19 items for the Diet which three point likert type Diet Information Scale ofHaemodialysis Patients (DISHP) and a Behaviour Scale with 18 items for the Diet which 5 point likert type Diet Behaviour Scale ofHaemodialysis Patients (DBSHP). Cronbach alpha coefficient for DISHP and DBSHP were found respectively as 0,86 and 0,73. In the second stage; 80 haemodialysis patients were divided into two groups as control and experimental groups; experimental group patients were educated on diet and fluid control by the researcher nurse. After the education given; it was determined that the last test points of DISHP and DBSHP increased significantly in comparison with the preliminary test points (p<0,05) while serum sodium amount, the weight between two dialysis and diastolic blood pressure significantly decreased (p<0,05). It was determined that as long as haemodialysis age increases in the experimental group, there is a positive correlation between preliminary test DISHP and DBSHP points (p<0,05). And it was also determined that in control group DISHP and DBSHP last test points significantly decreased compared with the preliminary test points (p<0,05). As a result; it was determined that the education given to the haemodialysis patients by the nurse has a positive effect in nutrition management. It is suggestible that haemodialysis patients should be educated regularly by the nurse in order to create a positive behavioural change about diet and fluid restriction.
