Publication: Serebral palsi’li çocuklarda simüle hippoterapi eğitiminin etkinliği
Abstract
ÖZETAmaç: Simüle hippoterapi eğitiminin Serebral Palsi’li çocuklar üzerindeki etkinliğini araştırmaktır.Gereç ve Yöntem: Çalışmaya 5-15 yaş arası, 30 SP’li (17 hemiparetik, 13 diparetik) çocuk alındı. Hippoterapi simülasyon eğitimi için Horse Riding Simülatör cihazı (HRS) kullanıldı. Çocuklar 8 hafta, haftada 2 kez, 45 dakika Nörogelişimsel Tedavi (NGT) sonrasında 8 hafta, haftada 2 kez, 30 dakika HRS ve 15 dakika NGT tedavisi aldı. Çocuklar tedavi öncesi, 8. ve 16. hafta sonunda PEHA, MAS, Myoton®PRO, KMFÖ-88, GEÖ, Wee-FIM, Pedalo® Sensamove Denge Testi, PDS ve WinTrack Yürüme Platformu ile değerlendirildi. Bulgular: HRS+NGT yöntemi sonrasında başlangıca ve NGT yaklaşımına göre çocukların KMFÖ-88, Wee-FIM, GEÖ, PDS, Pedalo® ve yürüme parametreleri sonuçlarında istatistiksel olarak anlamlı bir artış olduğu, MAS sonuçlarında anlamlı bir azalış olduğu (p<0,05), Myoton®Pro sonuçlarının ise değişmediği bulundu.Sonuçlar: Nörogelişimsel Tedavi yaklaşımıyla birlikte yapılan simüle hippoterapi eğitiminin SP’li çocukların spastisitesinin azaltılmasında, alt ekstremite hareketlerinin, kaba motor fonksiyonlarının, oturma postural kontrolünün, oturma ve ayakta durma dengesinin, yürüme fonksiyonlarının ve günlük yaşam aktivitelerinin geliştirilmesinde daha etkili olduğu belirlendi.

Children were assessed pre-treatment, at the end of 8th and 16th week by ROM, MAS, Myoton®Pro, GMFM-88, Wee-FIM, TIS, Pedalo® Sensamove Balance Test, PBS and WinTrack Platform.Findings: HRS+NDT method, according to the initial and NDT approach, showed a statistically significant increase in the results of GMFM-88, Wee-FIM, TIS, PDS, Pedalo® and gait parameters and decrease in results of MAS (p<0,05), however, results of Myoton®Pro were unchanged.Results: Simulated hippotherapy training is better at decreasing spasticity and improving gross motor function, lower extremity function, sitting postural control, sitting and standing balance, gait functions and activity of daily living when NDT treatment children with cerebral palsy.
