Publication: Primer açık açılı glokom hastalarında brimonidin tartrat %0.2 monoterapisi ve bimatoprost %0.03 monoterapisinin göz içi basıncı üzerine ve glokom evreleme sistemi 2 (Brusini Kriterleri) ile görme alanı üzerine etkilerinin değerlendirilmesi
Abstract
Amaç: Bu çalışma primer açık açılı glokom hastalarında brimonidin tartrat %0.2 monoterapisi ve bimatoprost %0.03 monoterapisinin göz içi basıncı üzerine ve Glokom Evreleme Sistemi 2 ( Brusini kriterleri) ile görme alanı üzerine etkilerini değerlendirmek amacı ile gerçekleştirildi. Gereç ve Yöntem: Altmışyedi primer açık açılı glokom hastasının bir yıllık takip süresi değerlendirmeye alındı ve bu süre boyunca 67 hastanın 35 tanesi her iki gözüne bimatoprost %0.03 monoterapisi ve diğer 32 tanesi her iki gözüne brimonidin tartrat %0.2 monoterapisi almıştı. Hastaların sağ ve sol gözlerinin başlangıç, 1.ay, 3.ay, 6. ay, 1.yıl GİB’leri , santral kornea kalınlıkları, başlangıç görme alanı ve 1. yıl görme alanı bulguları değerlendirmeye alındı. Hastaların başlangıç ve 1. yıl görme alanı bulguları GSS 2’ye göre evrelendirildi. Bulgular: Brimonidin tartrat %0.2 ve bimatoprost %0.03’ün sağ göz ve sol gözde başlangıca göre bir senelik takip süresinde GİB düşürücü etkisi anlamlı olarak bulundu ( p<0.001). Bimatoprost %0.03’ün sağ gözde ve sol gözde GİB düşürücü etkisi brimonidin tartrat %0.2’den daha fazla olarak bulundu. Brimonidin tartrat %0.2, bimatoprost %0.03’e göre sağ gözde ( %65.6’ya %28.6), sol gözde ( %71.9’a %40) ve bütün olgularda ( %68.8’e %34.3) görme alanında daha fazla evre iyileşmesi sağlıyordu. Bimatoprost %0.03, brimonidin tartrat %0.2’ye göre, sağ gözde ( %54.3’e %15.6), sol gözde ( %45.7’ye %18.8) ve bütün olgularda ( %50’ye %17.2) görme alanında daha fazla evre stabilizasyonu sağlıyordu. Sonuç: Brimonidin tartrat %0.2 ve bimatoprost %0.03’ün bir senelik takipte GİB’i düşürmede etkin ilaçlar old
Objective: This study was established in order to assess the effects of the bimonidine tartrate 0.2% monotherapy and bimatoprost 0.03% monotherapy on intraocular pressure and on visual field with Glaucoma Staging System 2 ( Brusini criteria) in primary open angle glaucoma patients. Materials and Methods: One-year follow-up duration of the 67 primary open angle glaucoma patients was assessed and during this period, thirty five of sixty seven patients had received bimatoprost 0.03% monotherapy to both eyes and other thirty two patients had received brimonidine tartrate 0.2% monotherapy to both eyes. The intraocular pressures of the right and left eyes of the patients, their central cornea thicknesses, initial visual field findings and the visual field findings at the end of the first year were assessed. The initial visual field findings and the visual field findings at the end of the first year were staged with respect to GSS 2. Results: The intraocular pressure reducing effect of the brimonidine tartrate 0.2% and bimatoprost 0.03% at the right and left eye was found significant at the end of the one-year follow-up period compared to the initial effect ( p<0.001). The reduction effect of bimatoprost 0.03% in the intraocular pressure at the right and left eye was found more than that of brimonidine tartrate 0.2%. Brimonidine tartrate 0.2% , was made more visual field improvement at the right eye ( 65.6% vs 28.6%), left eye ( 71.9% vs 40%) and in all cases ( 68.8% vs 34.3%) than bimatoprost 0.03%. Bimatoprost 0.03%, was made more visual field stabilization at the right eye ( 54.3% vs 15.6%), left eye ( 45.7% vs 18.8%) and in all cases ( 50% vs 17.2%) than brimonidine tartrate 0.2%. Conclusion: It was observed that brimonidine tartrate 0.2%.and bimatoprost 0.03% were effective drugs in reducing intraocular pressure during the one-year follow-up. It was understood that the effect of bimatoprost 0.03% in stabilizing visual field stage can be originated from its activity of in reducing intraocular pressure and, that the effect of brimonidine tartrate 0.2% in improving visual field stage can be originated from another activity such as its neuroprotective characteristic instead of its activity in reducing intraocular pressure.
