Cerrahi tedavide laser kullanımı ve İstanbul ili hastanelerindeki ameliyathanelerde laser kullanımına yönelik alınan önlemlerin saptanması

Abstract

Araştırma, cerrahi tedavide laser kullanımı ve İstanbul İli hastanelerindeki ameliyathanelerde laser kullanımına yönelik alınan önlemleri saptamak amacıyla tanımlayıcı olarak gerçekleştirilmiştir. Araştırmanın evrenini İstanbul ili hastaneleri oluşturmuş, örnekleminin oluşturulmasında İstanbul Tabip Odası ve İstanbul İl Sağlık Müdürlüğü'nün hazırladığı 2001 İstanbul Sağlık Rehberi ve Sağlık Bakanlığı Tedavi Hizmetleri Genel Müdürlüğü'nün hazırladığı Yataklı Tedavi Kurumları İstatistik Yıllığı kullanılmıştır. Araştırmanın evrenini, İstanbul İli içerisinde farklı statülerde hizmet veren, biri askeri 4 üniversite, 11 S.S.K, 32 devlet ve 179 özel (Enstitü, İçişleri Bakanlığı, PTT, Vakıf ve Azınlık Hastaneleri dahil ) olmak üzere toplam 226 hastanenin ameliyathaneleri oluşturmuştur. Bu hastanelerin tümüne ulaşılması hedeflenmesine karşın çalışma için izin alınamayan ya da görüşmeyi kabul etmeyen toplam 75 hastane araştırma kapsamından çıkarılmıştır. Bu nedenle araştırmanın örneklemini, %133 özel, 4 üniversite, 6 S.S.K ve 8 devlet hastanesi olmak üzere toplam 151 hastanenin ameliyathaneleri oluşturmuştur. Bu hastanelerden %27.1 (n=41)' i laser teknolojisini kullanmakta olup %72.9 (n=110)' u ise laser teknolojisini kullanmamaktadır. Araştırma verileri bilgi formu kullanılarak, araştırmacı tarafından yüz yüze görüşme yöntemiyle yapılmıştır. Bilgi formunun doldurulmasında ameliyathane başhemşire / sorumlu hemşireleriyle görüşülmeye dikkat edilmiştir. Veriler SPSS programında değerlendirilmiş, yüzdelik değerler ve Fisher kesin ki- kare testi kullanılmıştır.Araştırmadan elde edilen verilere göre, laser teknolojisi kullanımının özel hastanelerde daha fazla olduğu ve laser teknolojisinin kullanımının ekonomik faktörlerden etkilendiği ve alınması gereken önlemlerin yetersiz olduğu saptanmıştır. Elde edilen verilere yönelik uygun önerilerde bulunulmuştur. USE OF LASER IN SURGERY AND THE DETERMINATION OF PRECAUTIONS CONCERNING THE USE OF LASER IN OPERATING ROOMS OF THE HOSPITALS IN ISTANBUL

The study was carried out to determine the use of laser in surgical treatment and to determine the precautions to be taken in the operating rooms where the laser has been used in the hospitals in Istanbul and was planned descriptively. The population of the study was composed of the hospitals in Istanbul; and during constituting the sample, 2001 Istanbul Health Guide which was prepared by Istanbul Chamber of Physicians and Province Health Directorate of Istanbul, and Statistical Annual of Treatment Institutions with Beds which was prepared by Ministry of Health - General Directorate of Treatment Services was used. The populatıon of the study, was composed of the operating rooms of 11 Socıal Institution, 32 state, 4 university and 12 military totally 226 hospitals including institute, Ministry of Inferior, PTT, endowment, minority hospitals) which have been serving in different statutes within the province Istanbul. Although it was aimed to be reached to all of those hospitals, total 75 hospitals that did not accept the negatiation and that the permissions were not provided, were exluded from the study. Because of this reason, the sample of the study was composed of 133 private hospitals, 8 state hospitals, 6 social security institution hospitals, 4 university hospitals (one of them is the military hospital). Totally 151 hospitals were investigated and the results found that % 27.1 (n=41) used laser technology and %72.9 (n=110) did not use. The data of the study were collected through mutual interviews with the head nurse / charge nurse of the operating rooms with the information form prepared by the recearher. Percentage and Fischer's exact chi square tests were used in the assessment of the data. According to the results of the study, it was determined that the use of laser technology was more in the private hospitals and that the use of laser technology was influenced from the economical factors and the precautions to be taken were insufficient. Appropriate suggestions were made according to the results.
