Abstract
Bilişsel Davranışçı Terapi ile Tedavi Edilen Vajinismuslu Kadınlarda ve Eşlerinde Tedavi Öncesi ve Sonrası Benlik Saygısı, Özyeterlilik ve Yaşam Kalitesi Düzeyleri Amaç; Vajinismuslu kadınların ve eşlerinin tedavi başında benlik saygısı, özyeterlilik ve yaşam kalitesi düzeylerini ölçmek ve tedavi sonunda bu düzeylerdeki değişimi değerlendirmek. Ek olarak tedavinin tamamlanmasını yordayabilecek etmenleri belirlemek. Yöntem; MÜPEAH‟ne ve BRSHH ikinci basamak tedavi ünitesine vajinismus ön tanısıyla baĢvuran 36 çift çalıĢmaya dahil edilmiĢtir. Çiftlerin sosyodemografik verileri ve cinsel yaĢam özellikleri kiĢisel bilgi formuyla değerlendirilerek ilk seansta vajinismuslu kadına ve eĢine Genel Özyeterlilik Ölçeği (GÖÖ), Rosenberg Benlik Saygısı Ölçeği-benlik saygısı alt ölçeği (RBSÖ-ben), Coopersmith Benlik Saygısı Ölçeği (CBSÖ), Dünya Sağlık Örgütü YaĢam Kalitesi Ölçeği Türkçe Kısa Form (WHOQOL-BREF-TR) ve Golombok-Rust Cinsel Doyum Ölçeği (GRCDÖ) uygulanmıĢ olup tedaviyi tamamlayan çiftlere aynı ölçekler tekrar uygulanmıĢtır. Bulgular; Tedaviyi tamamlayan vajinismuslu kadınlar ve eĢlerinde tedavi sonunda benlik saygısı, yaĢam kalitesi, özyeterlilik ve cinsel doyum düzeylerinde anlamlı ve olumlu değiĢimler olduğu tespit edilmiĢtir. Ek olarak tedavinin ikinci seansında kadının kendi parmağıyla vajinasına giriĢ yapabildiğini bildirmesinin ve kadının tedavinin baĢında yüksek WHOQOL-BREF-TR psikolojik alt ölçek puanlarına sahip olmasının tedavinin tamamlanmasında yordayıcı olduğu görülmüĢtür. Sonuç; Vajinismus kadının ve dolayısıyla çiftin yaĢam kalitesini, benlik saygısını, özyeterliliğini ve cinsel doyumunu etkileyen bir cinsel iĢlev bozukluğudur. BiliĢsel-DavranıĢçı terapi ile birlikte sadece cinsel iĢlevlerde değil aynı zamanda yaĢam kalitesi, benlik saygısı ve özyeterlilikte de anlamlı ve olumlu yönde değiĢiklikler olmaktadır. Bu yüzden BDT gibi pek çok psikolojik ve davranıĢçı müdahaleyi içinde barındıran bütüncül yaklaĢımların vajinismuslu kadınların ve eĢlerinin tedavisinde faydalı olabileceği düĢünülmüĢtür. viii
Self-Esteem, Self-Efficacy and Quality of Life Levels Among Women with Vaginismus and Their Spouses whom Treated with Cognitive-Behavioral Therapy Aim; to assess self-efficacy, self-esteem and quality of life measures in the beginning and at the end of treatment among women with vaginismus and their spouses. In addition to assess factors that might be predictive of vaginismus treatment completion. Method; Thirty six couples with possible vaginismus diagnosis who were admitted to Marmara University Faculty of Medicine Psychiatry Department and Bakırköy Psychiatry and Neurology Hospital second step treatment facility were included. At the first session, couples sociodemographic data and sexual life characteristics assessed with personal information form. After that General Self-Efficacy Scale, Rosenberg Self-Esteem Scale-self esteem subscale, Coopersmith Self-Esteem Scale, World Health Organization Quality of Life Scale Turkish Brief Form and Golombok-Rust Sexual Satisfaction Scale were applied to women with vaginismus and their spouses. The participants, who completed the study were given the same scales again. Results; At the end of the treatment there were significant and positive changes in self-esteem, quality of life, self-efficacy and sexual satisfaction in women with vaginismus and their spouses who had completed the treatment. In addition, reported self-finger insertion by the woman to her vagina at the second session and woman‟s high levels of psychology subscale points in WHOQOL-BREF-TR at the beginning of treatment seemed to be predictive of vaginismus treatment completion. Conclusions; Vaginismus is a sexual dysfunction that affects women‟s thereby the couple‟s quality of life, self-esteem, self-efficacy and sexual satisfaction. It seems CBT may lead significant and positive changes not only in sexual function but also in quality of life, self esteem and self efficacy.
