Akut lenfoblastik lösemili çocukta mantar infeksiyonu ile ayırt edilemeyen akciğer apsesi: Olgu sunumu

Abstract

Kemoterapi sonrasında febril nötropenik hastalarda birçok fırsatçı enfeksiyon meydana gelmektedir. Febril nötropenik hastalarda akciğerde kitle saptandığında ilk olarak tümör metastazı ve mantar enfeksiyonları akla gelmektedir. Kitle izlenimi veren akciğer apseleri ise son derece ender bir durumdur. Bu makalede febril nötropenik bir hastada kitle görünümü veren mantar topu ve tümör metastazı ile karışan ESBL (-) E.coli ye bağlı olarak apse gelişen hastayı sunu- yoruz. Hasta uygun antimikrobiyal tedavi ve cerrrahi girişim ile 52 günde şifa ile taburcu edilmiştir. Febril nötropenisi olup, akciğerinde kitle lezyonu tespit edilen hastalarda mantar topu, tümör metastazı ile birlikte apselerin erken dönemlerinde benzer görünümde olabileceğine dikkat çekmek istedik.

Many opportunistic infections occur in febrile neutropenic patients after chemotherapy. When mass lesions are found in the lungs of febrile neutropenic patients the first things that come to mind are tumor metastasis and yeast infections. Lung abscess that presents with the appearance of a mass lesion is a very rarely seen condition. This paper presents a febrile neutropenic patient with an abscess caused by ESBL (-) E.coli with mass appearance similar to fungal ball and tumor metastasis. After antimicrobial treatment and 52 days after surgical intervention the patient was discharged. We wish to draw attention to the fact that in patients with pulmonary mass lesions and febrile neutropenia, fungal balls, tumor metastasis and abscesses have a similar appe - arance in the early phase of the disease.
