Publication: Yeni mezun hemşirelerde gerçeklik şokunun nitel açıdan incelenmesi ve gerçeklik şoku ölçeğinin geliştirilmesi
Abstract
Amaç: Çalışma, yeni mezun hemşirelerde gerçeklik şokunu araştırmak amacıyla tanımlayıcı, gerçeklik şoku ölçeğinin geliştirilmesi amacıyla metodolojik tasarımda gerçekleştirilmiştir. Gereç ve Yöntem: Nitel aşamada Sağlık Bakanlığı, Özel ve Üniversite olmak üzere beş hastanede görevli mesleki deneyimi en fazla iki yıl olan 57 hemşireden, niceliksel aşama için aynı özelliklere sahip sekiz hastanede görevli 810 hemşireden veri toplanmıştır. Niteliksel bölümde yarı yapılandırılmış görüşme formu, geçerlilik ve güvenilirlik için ölçek taslağı kullanılmıştır. Verilerin analizinde; ilk aşamada içerik analizi, ikinci aşamada ölçek geçerlik ve güvenirliğinde kullanılan analizler kullanılmıştır.Bulgular: Yeni mezun hemşirelerde gerçeklik şokunda tanımlanan temalar; mesleki bilgi, çalışma ortamı, ilişkiler, rol ve sorumluluklardır. Hemşireler gerçeklik şokunu beklentilerin karşılanmaması ve mesleğe geçiş sürecinde yaşanan zorluk olarak tanımlamıştır. Ölçek geliştirme aşamasında; bireysel derinlemesine görüşmelerle 91 maddelik ölçek madde havuzu oluşturulmuştur. Kapsam geçerliliği sonrası 87 maddeye düşen taslak ölçeğin geçerlik güvenirlik çalışması yapılmıştır. Geçerlik güvenirlik çalışması sonucunda 47 maddelik 4 alt boyutundan (İlişkiler ve İşbirliği, Mesleki Bilgi, Sorumluluk ve Performans) oluşan, iç tutarlılığı α=0,95 olan ve 5’li Likert tipi ile değerlendirilen bir ölçek oluşturulmuştur.Sonuç: Yeni mezun hemşirelerin geçiş sürecini kolaylaştırmak için desteğe gereksinim duydukları belirlenmiştir. Geliştirilen Gerçeklik Şoku Ölçeği ise yeni mezunun yaşadığı gerçeklik şokunu belirlemede geçerli ve güvenilir bir ölçme aracı olarak kullanılabilir.
Aim: The study was conducted in methodological design to develop a descriptive reality shock scale in order to investigate reality shock in newly graduated nurses. Materials and Methods: In the qualitative phase, data were collected from 57 nurses with a maximum of two years of professional experience in five hospitals, namely Training and Research, Private and University, and 810 nurses working in eight hospitals with the same characteristics for the quantitative phase. In the qualitative part, the semi-structured interview form and the draft scale for the validity and reliability were used. For the analysis of the data; in the first stage, content analysis was used and in the second stage, the analyses used for the validity and reliability of the scale were used.Findings: Themes defined in reality shock in newly graduated nurses include professional knowledge, working environment, relationships, roles and responsibilities. Nurses defined reality shock as not meeting expectations and difficulty in transition to profession. During the scale development stage; a scale pool of 91 items was created through individual in-depth interviews. After the validity of the scope, validity and reliability study of the draft scale reduced to 87 items was conducted. As a result of the validity and reliability study, a scale consisting of 4 sub-dimensions of 47 items (Relationships and Collaboration, Professional Knowledge, Responsibility and Performance), with internal consistency α = 0,95 and a 5-point Likert type, was developed.Conclusions: It was determined that newly graduated nurses needed support to facilitate the transition process. Furthermore, the Reality Shock Scale developed is a valid and reliable measurement tool for determining the reality shock experienced by the new graduate.
