Farklı implantlar ve tutucu elemanlar kullanıldığında implant üssü tam protezlerin ısırma kuvvetlerinin ve çiğneme kaslarının aktivitelerinde oluştudukları değişikliklerin EMG (elektromiyografi) ile incelenmesi

Abstract

Dissiz hastaların protetik tedavisinde uygulanan total protezlerle her zaman hasta memnuniyetini saglamak mümkün degildir. Ayrıca hastaların yası ilerledikçe, dis kaybının bir sonucu olarak, kret rezorbsiyonu, çigneme etkinliginde azalma, kas dengesinde degisim, dikey boyutun düsmesi, estetik ve fonetik yetersizlik gözlemlenir. Bunun sonucunda da yapılan protezlerde tutuculuk ve stabilite saglarken problemler ortaya çıkar. Bu problemlerin üstesinden gelebilmek için total dissizlige sahip bireylerde implant destekli hareketli protezlerin uygulanımı son yıllarda oldukça popüler olmustur. Çalısmaya Marmara Üniversitesi Dishekimligi Fakültesi Protetik Dis Tedavisi Anabilim Dalına basvuran, fiziksel ve psikolojik olarak konvansiyonel cerrahi ve protetik islemleri tolere edebilecek, yas ortalaması 60,1 olan, 26 hasta dahil edildi. Hastaların intraforaminal bölgesine anatomik sartlara göre 2 ve 4 implant yerlestirildi. Osseointegrasyonun tamamlanmasından sonra implant destekli total protezler yapılarak hastalara teslim edildi. Protezlerin uygulanmasından hemen sonra hastalar 1 hafta, 6, 12 ve 24 aylık periyotlarla kontrole çagrıldı. Kontrol seanslarında implantlar klinik ve radyografik açıdan degerlendirildi. Klinik degerlendirmelerde plak indeksi, diseti cebi derinligi, sulkus kanama indeksi, mobilite indeksi ve perküsyon degerlendirmesi yapıldı. Ayrıca hasta memnuniyeti, uygulanan tedavi yöntemi ve yapılan protezlerle saglanan estetik, çigneme etkinligi, konusma, protezlerin tutuculuk ve stabilitesi, temizleme kolaylıgı ile ilgili soruları içeren anket formuyla degerlendirildi. Hastalar protezlerini sorunsuz olarak kullanmaya basladıktan sonra masseter kasına baglanan tek kullanımlık yüzeyel elektrotlarla 1’er gram fıstık yerken, 1’er gram sakız çignerken ve hazırlanan silikon kalıbı maksimum ısırma sırasında EMG ölçümleri alındı. Ölçümler, atasmanlar söküldükten sonra protez implantlara baglı olmadan tekrarlandı. Ayrıca protezin doku yüzeyine doku düzenleyici uygulanarak tekrar aynı prosedürlerle ölçümler alındı. Bu çalısmada total protezlerinden memnun olmayan bireylere implant destekli hareketli protezler uygulanarak hasta memnuniyeti degerlendirildi. mplant destegi ile tutuculukları arttırılan alt çene total protezlerde çigneme etkinligi EMG degerlendirilerek ölçüldü. Sonuçta, implantlar klinik ve radyografik açıdan basarılı, hasta memnuniyeti yüksek bulundu. EMG testleri sonucunda implant kullanımıyla masseter kas aktivitesinde belirgin bir artıs oldugu görüldü. EMG (ELECTROMYOGRAPHY) EXAMINATION OF BITING FORCES AND CHEWING MUSCLES ACTIVITY CHANGES OF IMPLANT SUPPORTED OVERDENTURES WHEN USING DIFFERENT IMPLANTS AND RETANTIVE UNITS

Satisfaction of edentulous patients with complete denture is not always possible. As a result of tooth lost with aging, in older patients ridge resorption, decreasing in chewing efficiency, changing of muscle balance, losing vertical height, esthetic and fonetic inadequacy is observed. Because of these reasons while maintaining retention and stability of new dentures, some problems will be occured. To overcome of these problems of edentulous patients, it has been very popular to use implant supported removable dentures. 26 patients who are accepted physically and psychologically conventional for surgical and prosthodontic processes, applicated at Marmara University School Of Dentistry, Department of Prosthodontics were included to this study. The average age of these patients was 60,1. According to anatomical conditions, 2 or 4 implants are inserted to intraforaminal region. After osseointegration, patients were treated with implant supported complete dentures. Patients were called for recalls of 6, 12, 24 months of periods. During these recalls implants were examined clinically and radiographically. In clinical evaluations plaque index, periodontal pocket depth, sulcus bleeding index and percussion examinations were done. The patients’ own appreciation of the implant and the prosthetic therapy were also evaluated. Patients' opinions on oral functions, mastication, phonetics, oral hygiene, chewing comfort and esthetics, were evaluated by means of a questionnaire. After patients began using their dentures without any problems, they were evaluated with disposible superficial electrods on masseter muscle while chewing 1 gram of peanuts and 1 gram of chewing gum and silicone that is for maximum bite were evaluated by EMG. Measurements were repeated after pulling out attachments while dentures were not on the implants. Furthermore, tissue conditioner was coated on the tissue surface of the denture and measurements were repeated with the same procedures. In this study, patients, who were unsatisfied with their dentures, were treated with implant supported overdentures and evaluated by means of patients satisfaction. Retention increased lower complete dentures with implant support were evaluated by EMG for their biting efficiency. As a result, the implants were clinically and radiographicaly healthy. The masseter muscle activity significantlly has incresed when the atachments were used.
