İşitsel uyarılmış P1-N1-P2 kortikal potansiyellerle eşik incelemesi

Abstract

İŞİTSEL UYARILMIŞ P1-N1-P2 KORTİKAL POTANSİYELLERLE EŞİK İNCELEMESİ Bu çalışmanın amacı, normal işitmeli ve sensörinöral işitme kayıplı yetişkinlerde işitsel uyarılmış geç kortikal cevaplar (Auditory Late Response-ALR) ve klasik davranışsal tekniklerle elde edilen işitme eşiği sonuçlarını karşılaştırmak ve bu iki testin birbiri ile ne kadar uyumluluk gösterdiğini araştırmaktır. Bunun için çalışmaya 18 normal işiten (33 kulak), 56 işitme kayıplı (112 kulak) olmak üzere toplam 74 yetişkin (145 kulak) alınmış ve 500, 1000, 2000 ve 4000 Hz frekanslarında elde edilen ALR ve davranım eşikleri karşılaştırılmıştır. Normal işitenlerde 1000 ve 2000 Hz’ de orta, 500 ve 4000 Hz’ de zayıf derecede bir korelasyon elde edilirken, işitme kayıplılarda tüm frekanslarda yüksek derecede bir korelasyon elde edilmiştir. ALR ve davranım eşik ortalamaları arasındaki fark değerleri incelendiğinde ise bu değerlerin normal işitenlerde pozitif, işitme kayıplılarda ise negatif değerler olduğu görülmüştür. Bu durum ALR eşik ortalamalarının normal işitenlerde saf ses davranım eşik ortalamalarından yüksek, işitme kayıplılarda ise düşük olduğunu göstermektedir. Elde edilen en büyük ortalama fark değeri normal işitenlerde 4.1 dB, işitme kayıplılarda ise 7.1 dB olup, eldilen bu farklar klinik açıdan anlamlı olarak edilmemiştir. Sonuç olarak; ALR’ nin 500, 1000, 2000 ve 4000 Hz frekanslarında saf ses davranım eşikleri ile normal ve işitme kayıplı kişilerde yüksek oranda uyumluluk gösterdiği görülmüştür. Bu nedenle ALR’ yi, davranım eşiklerine kooperasyonu olmayan çocuk ve yetişkinlerde, adli vakalarda ve nonorganik işitme kayıplarında eşik yordamak üzere kullanılabilecek uygun ve güvenilir bir test olarak önermek mümkündür. THRESHOLD ESTIMATION WITH THE P1-N1-P2 CORTICAL AUDITORY EVOKED POTENTIALS The aim of the study was to compare the hearing threshold levels obtained by cortical auditory evoked responses and classic behavioral audiometry in adults with normal hearing and sensorineural hearing loss and to study the compatibility of these two tests. In this study 74 adults consisting of 18 normal hearing and 56 hearing impaired people were evaluated and hearing threshold levels obtained with both tests at 500, 1000, 2000 and 4000 Hz frequencies were compared. The correlation between ALR and behavioral thresholds was found to be modarete in adults with normal hearing. On the other side in adults with hearing loss the correlation was found to be high at all of the tested frequencies. The mean differences values between ALR and pure tone behavioral thresholds were found to be positive in normal hearing people and negative in hearing disorders people. These data have shown that in adults with normal hearing, ALR thresholds were higher than pure tone behavioral thresholds. In contrast, in adults with sensorineural hearing loss, ALR thresholds were lower than pure tone behavioral thresholds. The biggest mean differences values were 4.1 dB in normal hearing people and 7.2 dB in hearing disorders people. These differances were found to be not significant for clinically in both groups. As a result, ALRs have showed good correlations with pure tone behavioral thresholds and it may be suggested as a reliable and accurate test to estimate pure tone thresholds in uncooperative adults and children, forensic cases and subjects with nonorganic hearing loss.
