Publication: Tip 2 diyabetlilerde planlı diyabet eğitiminin metabolik kontrol değişkenleri üzerine etkisi
Abstract
1.ÖZET Amaç: Araştırma; WHO kriterlerine göre tip 2 diyabet tanısı almış olan, insüline bağımlı olmayan diyabetlilere yapılan planlı diyabet eğitiminin, metabolik kontrol değişkenleri üzerine etkisini belirlemek amacıyla müdahale araştırması olarak planlanmıştır. Gereç-Yöntem: Çalışma, Ocak-Aralık 2002 tarihleri arasında Trakya Üniversitesi Tıp Fakültesi Endokrin polikliniğinde yürütülen, belirtilen kriterlere uygun seçilen 74 diyabetliyle gerçekleşmiştir. Veri toplama aracı olarak, 'Diyabetli tanılama formu', 'metabolik kontrol değişkenler formu' ve 'İç tutarlılığı yapılmış eğitim içeriğine uygun bilgi formu' kullanılmıştır. Grubun, metabolik kontrol düzeylerini değerlendirmek için; arteriyal kan basıncı, HbA1c, kolesterol, trigliserid, HDL, LDL, bel-kalça çevresi ve beden kitle indeksi ölçümleri yapılmıştır. Araştırmanın ilk 4 aylık süresinde, eğitim programının ve içeriğinin hazırlanması amacıyla literatür incelenmiş, materyal oluşturulmuştur. Sonraki 3 ay içersinde araştırmaya katılan diyabetlilerin, demografik, sosyo-ekonomik, eğitim durumları v.b. değişkenleri dikkate alınarak, 5 grup halinde eğitim verilmiştir. Her bir grup için 90'ar dk'lık 4 oturumda eğitim yapılmıştır. Eğitimin sonunda tüm konuları içeren 'Tip 2 Diyabetle Yaşam Kitapçığı' verilmiştir. Eğitim öncesi ve eğitimden 3 ay sonra, bilgi formu aracılığıyla grubun diyabet hastalığı hakkında bilgi puanına ilişkin verileri toplanmıştır. Bulgular ve Tartışma: Grubun yaş ortalaması 53,09±7,59 yıldır. Araştırma kapsamındaki diyabetlilerin, öğrenim durumları düşük, ekonomik durumları yeterli bulunmuştur. Üç aylık izlem süresinin sonunda diyabetlilerin metabolik kontrol değişkenlerinden HbA1c, lipitler ve arteriyal kan basıncı değerlerinde anlamlı değişiklikler saptanmıştır (p<0,05). Eğitim öncesi ve eğitim programından 3 ay sonra diyabetlilerin bilgi puanlarında istatistiksel olarak anlamlı fark bulunmuştur (p<0,05). Sonuç:Diyabetlilere sürekli danışmanlık sağlanarak belirli periyotlarla yapılacak planlı diyabet eğitiminin, metabolik kontrol değerlerinde anlamlı farklar yaratacağı, fakat eğitim ile metabolik kontrol değişkenleri arasındaki ilişkinin doğrulanabilmesi için daha uzun süreye gereksinim olduğu düşünülmektedir. 2.
THE EFFECTS OF A PLANNED DIABETES EDUCATION ON METABOLIC CONTROL VARIABLES OF TYPE 2 DIABETEC PATIENTS Purpose: Study has been desingned as intervention in order to determine effect of planned diabet training given to diabetics, who had diagnosis of diabet type 2 according to WHO criteria and are not addicted to insulin, on metabolic control variables. Materials and Method: Study has been performed on 74 diabetics who were chosen according to specified criteria, conducted in Trakya University Medicine Faculty Endocrin Clinics between January-December 2002. As a data collecting tool, Diagnostic form for diabetics, metabolic control variables form and information form appropriate to training content with inside consistency have been employed.In order to evaulate metabolic control level of group; easurementssuch as arterial blood level, HbA1c, cholesterol, trigliserid, HDL, LDL, waist-hip surroundings and body mass index have been conducted. During the first 4 months of period of research, literature has been examined in order to prepare training programme and its content, and materials have been established. For the following 3 months, training has been given in five groups, taking into consideration variables such as demographic, socio-economic, education, status etc. For each group, training has been given in four sessions, each lasting 90 minutes. At the end of the training, Booklet for living with Diabet Type 2 including all subjects has been given to those. Prior to training and 3 months after training, data regarding information scores related to diabetic disasese of the group has been collected through information form. Findigs and Discussion: Mean age of the group was 53.09±7.59 years. It was seen that education status of the diabetics participated to the research was low, and that their economical status was sufficient. At the end of 3 months of following period, significant changes were determined on HbA1c, lipids, and arterial blood pressure values from metabolic control variables of diabetics (p<0.05). Statistically significant differences have been observed in information scores of diabetics prior to training and 3 months after the training (p<0.005). Results: It was concluded that planned diabetic training to be performed within certain periods providing continuous consultancy to diabetics would create significant differecences on metabolic control values, however that longer time would be required in order to verify relation between training and metabolic control variables.
