Publication: Smoking cessation campaign as a public health policy : an investigation on the political and medical nature of smoking
Abstract
Türkiye, uluslararası ilk tütün kontrol anlaşması olan Tütün Kontrolü Çerçeve Sözleşmesini 2004 yılında imzalamış ve anlaşmadaki tüm önlemleri yerine getiren üçüncü ülke olmuştur. Çeşitli tüketim kısıtlamalarıyla başlayan bu önlemler sigara kullanıcılarına ücretsiz tedavi planları ve ilaçlar sunulmasına kadar genişletilmiştir. Bu tezde, sigara bırakma tedavisi hizmeti verilmesi amacıyla devlet hastanelerinde kurulmuş kliniklerinde yaptığım saha araştrmasından faydalanarak yeni halk sağlığı pratiklerinin Türkiye’de nasıl benimsendiğini inceliyorum. Kliniklerde benimsenen tedavi yöntemleri ve söylemlerin, kendi sağlığının sorumluluğunu yüklenen ve kendi kendini yöneten bireyler ütmekte nasıl etkili olduğunu sorusunu cevaplamaktayım. Halk sağlığının geliştirilmesi konusunda sigara bırakma kliniklerinin getirdiği yeni perspektif ve pratikleri analiz ederek bu kliniklerin neoliberal bireylerin üretilmesindeki fonksiyonlarını analiz etmekteyim. Bunun yanında, bilim ve teknoloji çalışmalarından da faydalanarak sigaranın medikalleşmesi konusunda daha geniş bir perspektif sunuyorum.
Turkey has signed the first international agreement on tobacco control in 2004 and became the third country in the world who has implemented all the measures in the agreement. These measures vary from implementing bans on tobacco consumption to offering treatment plans and medicines for smokers. In this thesis, I explore the practices and perspectives adopted within the tobacco control measures. Benefiting from my fieldwork in smoking cessation clinics, which are established in state hospitals to help smokers in quitting I examine how the new public health regime is being adopted in Turkey. I answer the question of how discourses about smoking and treatment processes in clinics take an active role in producing self-governing, self-controlling subjectivities who take charge of their health. I analyze functions of cessation clinics in terms of their role in introducing new perspectives and practices in public health promotion and production of neoliberal subjectivities. Besides, drawing on Science and Technology Studies, I introduce a broader perspective to evaluate the medicalization of smoking.
