Publication: Hemşirelik hizmetlerinin hemodiyaliz hastalarının doyumu üzerine etkisi
Abstract
Bu çalışma, hemşirelik hizmetlerinin hemodiyaliz hastalarının doyumu üzerine etkilerini saptamak ve daha etkin, kaliteli bir hemşirelik sunumu sağlayarak; hastaların hemşirelik hizmetlerinden duydukları doyumu arttırmak amacıyla, metodolojik, tanımlayıcı ve karşılaştırmalı olarak planlanmıştır. Araştırmada; SERVQUAL ÖLÇEĞİ ve HASTA TANITICI ÖZELLİKLER FORMU kullanılmış olup, Servqual Ölçeği ile hastaların hemşirelik hizmetlerini değerlendirmeleri sağlanmıştır. Araştırma; İstanbul genelinde bulunan Devlet Hastaneleri, Sosyal Sigortalar Kurumu Hastaneleri, Üniversite Hastaneleri ve Özel Hemodiyaliz Ünitelerinde tedavi görmekte olan; 18-65 yaş arası, iletişim problemleri olmayan, Türkçe konuşabilen, okuma-yazma bilen, erkek-kadın, olasılıksız örneklem yöntemi ile seçilen ve her hastane grubu için yetmiş beşer hasta olmak üzere, toplam 300 hasta ile yapılmıştır. Servgual Ölçeği'nin hemodiyaliz hastalarında iç tutarlılığı anlamlı bulunmuş (r : 0.93 ve p<0.001), güvenilir bir ölçek olarak kullanılabilir olduğu saptanmıştır. Elde edilen veriler Cronbach ALPHA , Mann-Whitney U , t testi (student T) ve Kruskall- Wallis testleri ile değerlendirilmiştir. Çalışmaya katılan bireylerin yaş ortalaması 43 olup, %45,3'ü (136 kişi) kadın ve % 54,7'si (164 kişi) erkektir. Katılımcıların % 65,7'si (197 kişi) 12 aydan uzun süredir ve % 73,7'si (221 kişi) haftada 3 kez hemodiyalize girdiklerini belirtmiştir. % 40'ı (120 kişi) tedavi gördükleri diyaliz merkezini seçme nedeni olarak sağlık güvencesini göstermiştir. Çalışmanın sonucunda; hastaların hemşirelik hizmetlerinden memnun kalmadıkları ortaya çıkmıştır. İstatistiksel açıdan hastaların yaşı, cinsiyeti, eğitim düzeyi, medeni hali, sağlık güvencesi, hemodiyalize girme süresi ve haftalık seans sayısı, böbrek nakli olma durumu, daha önce başka bir merkezde diyalize girmeleri ve bu merkezde duydukları memnuniyetin, hemşirelik hizmetlerden aldıkları doyuma olan etkisi anlamlı bulunmamış olup, gelirlerinin giderlerini karşılama durumu ve hemodiyalize girdikleri hastane türüne göre anlamlı fark bulunmuştur.
THE EFFECT OF NURSE SERVİCES ON THE HEMODİALYSİS PATİENTS SATİSFACTİON The aim of this study was to evaluate the effect of the nurse services on the hemodialysis patients satisfaction, by increasing the quality of the services.This studied was planed methodological, determined and comparatived. Servqual measurement and patient advertiser form was used in order to evaluate the nurse services to the patients. The research was carried on 300 patients, 136 (43 %) females and 164 (54.7 %) males, aged betwen 18-65 years ( mean age: 53,3 years ), who could read and write Turkish, cooperative, male and female treated in The Social Insurance, State , Private Hospitals and Private Hemodialysis Units from Istanbul. 75 patient were selected from each type of hospital. 197 (65.7 %) of 300 patients were on hemodaialysis for more than 12 months, 221 ( 73.7 %) were dialysed 2 to 3 times a week. For 120 ( 40 %) of the patients, the reason of choosing the respective hemodialysis unit was the health insurance statu. The Servqual measurement validity was found to be significantly high (r:0,93 and p< 0.001) . Cronbach ALPHA, Mann- Whitney U, T student and Kruskall- Wallis tests were used as statistical methods. In conclusion, we found that the patients were not satisfied by the nurse services. The effect of the age or sex of the patient, education level, health insurance, the hemodialysis rate or lenght, kidney transplantation status, the history of treatment at another hemodialysis centre and the according satisfaction was found to be insignificant. Results were affected by the financial status and the type of the hospital used.
