Publication: Ameliyathane Hemşirelerinin Mesleki Profesyonelliklerinin İntraoperatif Bakım Kalitesine Etkisinin Değerlendirilmesi
Abstract
ÖZET Amaç: Bu çalışma, ameliyathane hemşirelerinin mesleki profesyonelliklerinin, verdikleri intraoperatif bakım kalitesine etkisini incelemek amacıyla tanımlayıcı olarak yapılmıştır. Gereç ve Yöntemler: Çalışma 1 Haziran-30 Kasım 2014 tarihleri arasında, Bursa ilinde bulunan iki hastanede yürütülmüş olup, araştırmanın verileri; "Hemşire ve Hasta Tanıtım Formları", "Kaliteli Perioperatif Hemşirelik Bakım Skalası" (KPHBS) ile "Mesleki Profesyonellik Tutum Envanteri" (MPTE) kullanılarak toplanmıştır. Çalışmanın örneklemini, 110 ameliyathane hemşiresi ve ameliyat olan 250 hasta oluşturmuştur. Veri analizinde, frekans (sıklık) ve yüzdelik dağılım, wilcoxon iki örnekli testi, t testi, Mann-Whitney U ve Kruskal Wallis testleri kullanılmış ve sonuçlar %95lik güven aralığında, anlamlılık değeri p<0,05 düzeyinde değerlendirilmiştir. Bulgular: Hemşirelerin mesleki profesyonellik tutum düzeyleri ile kaliteli perioperatif hemşirelik bakım düzeyi arasında pozitif yönde ileri derecede bir ilişki belirlenmiştir (p=0,000). Mesleki profesyonelliğin, KPHBS alt boyutlarından fiziksel bakım, bilgi verme, destek, çevre ve hemşirelik süreci alt boyutları ile ilişkisinin anlamlı olduğu (p=0,000), saygı (p=0,251) ve personel karakterleri (p=0,729) alt boyutları ile ilişkisinin anlamlı olmadığı belirlenmiştir. Hastaların KPHBS puan ortalamalarının hemşirelerin puan ortalamalarına göre daha yüksek ve farkın istatistiksel olarak ileri düzeyde anlamlı olduğu saptanmıştır (p=0,000). Sonuç: Bu çalışmada, ameliyathane hemşirelerinin mesleki profesyonelliklerinin yüksek olduğu ve mesleki profesyonelliklerinin intraoperatif bakım kalitesi ile ilişkili olduğu saptanmıştır. Hemşirelerin, mesleği profesyonelce uygulayamama nedeni olarak kurumsal eksikliklerin önemli etkisi olduğu sonucuna ulaşılmıştır. Kurumlar gerekli sayı ve nitelikte ekip sağlarken, malzeme gereksinimini de göz önünde bulundurarak kaliteli bakım verilmesine katkıda bulunabilir.

MMaatteerriiaall aanndd MMeetthhooddss:: The study was conducted between June 1stand November 30th-2014 in two hospitals located in Bursa. Data was col- lected using "Patient and Nurse Identification Form'', "Good Perioperative of Nursing Care Scale" (GPNCS) and "Inventory of Professional Attitude at Occupation". The sample included 110 operating room nurses and 250 patients who underwent surgery. Data were analyzed using frequency and percentage values, Wilcoxon two-sample test, t-test, Mann Whitney U test, and Kruskall Wallis test. Findings were evaluated according to a confidence interval of 95% and a significance level of p<0.05. RReessuullttss:: Level of occupational profes- sionalism was significantly and positively associated with good perioperative nursing care (p= 0.000). Oc- cupational professionalism significantly correlated with the physical care, providing information, support, environment, and nursing process subdimensions of good perioperative nursing care (p= 0.000); while it was not correlated with the respect (p= 0.251) and personnel features subdimensions (p= 0.729). Mean GPNCS scores of the patients was significantly higher than the mean scores of nurses (p= 0.000). CCoonncclluussiioonn:: In this study, it was determined that the operating room nurses had high levels of occupational professionalism and that their occupational professionalism was related to the quality of intraoperative care. It was indicated that the reason for not being able to exhibit occupational professionalism was organizational shortcomings. While providing an adequate team in terms of quality and quantity, organizations also should consider ma- terial needs in order to contribute to the provision of good care.
