P.Publication Open Access The Determination Of The Factors And Expectations Which Affect The Utilization of Health Services In_x000D_ Women: A Qualitative Research(2020-03-20) CÖBEK ÜNALAN, GÜLRU PEMRA; İkbal Hümay ARMAN;Elif OKUMUŞ;Nan Ye LAY;Kader TOPÇU;Özlem SARIKAYA;Pemra CÖBEK ÜNALANAim: To determine the behaviors, expectations and experiences about health-sickness of women from society, to evaluate the socio-cultural characteristics of thesebehaviors. Method: This research is qualitative, planned as “case study”. The research is sampled as goal-oriented maximum variation and 31 women included who arebetween 17-77 with different socio-cultural characteristics. Four focused interviews were conducted. Before the study, participants were informed and a questionnairewas used to question socio-demographic characteristics. The interviews were made at participants’ houses. One interviewer and two observers was ready in eachsession. The voice recordings and observers’ notes were trasncripted. No names have been transcribed. Each of the interviews were analyzed thematically, werecommonised to reach research findings. Results: According to the analysis, there are five theme topics. 1. Physician (professionalism) characteristics affecting thechoice of doctor (skills, attitude, gender, knowledge). 2. Expectations and behaviours about services related to women's health (afraid of the examination style,expectations of confidentiality and sympathy, prejudices). 3. Reasons for choosing health institutions (attitude of health workers, gender of doctor, hospital facilities,accessibility, prejudices, queue waiting). 4. Unmet health needs and thoughts about the causes (fear of malpractice, shyness, financial impossibilities, advancedappointments). 5. Attitudes on Family Health Centers (reasons for not using: lack of equipment, crowd, existing habits, doctor’s attitude, distrust to doctor'sknowledge). The participants repeated that dental health services are the most difficult to obtain. Conclusion: Socio-cultural characteristics, health services presentationand attitude of health workers influences expectations of individuals about utilization of health services. Therefore, when planning trainings of health workers, theapplicants' health perceptions affected by the socio-cultural characteristics, learning methods which allows the development of a positive attitude and improvescommunication skills should be considered. P., Samlı B., SARPER N., GÖKALP A. S.Amaç:Bu çalışma, annelerin süt çocuğu beslenmesi ve emzirme konusundaki bilgi ve uygulamalarının ortaya çıkarılmasını amaçlayan niteliksel bir araştırmadır. Çalışma grubunu 9-15 aylık bebeği olan ve farklı eğitim seviyesi, sosyal ve ekonomik yapıdan 40 anne oluşturmuştur. Yöntem:Çalışmanın verisi, yarı yapılandırılmış sorulardan oluşan bir akış formunun kullanıldığı derinlemesine görüşmeler ile toplanmıştır. Bulgular:Analiz sonrasında, görüşmeler sırasında annelerin ifade ettikleri şu ana temalar altında toplanmıştır:1- Annelerin anne sütü verme kararını etkileyen süreçler, 2-Anne sütü ile ilgili bilgi ve inanışlar, 3-Emzirme uygulamaları, 4-Annelerin ek besin başlama kararını etkileyen durumlar, 5-Annelerin ek besinlerle ilgili bilgi ve tutumları. Yurdumuzda emzirme alışkanlığı yaygındır. Çalışmamızda da annelerin tümü bebeklerini emzirmişlerdir. Ancak anne sütünün yeterliliği konusundaki endişeleri ve sosyal baskılar ek gıdalara erken başlanmasına neden olmaktadır. Eğitimler, anne sütü yeterliliğinin değerlendirilmesinde bebeğin kilo alımının esas belirteç olduğu ve anne sütünün tek başına 6 ay yeterli olduğu yönünde vurgu yapmalıdır. Ek gıdaların seçiminde ailelerin alım gücü etkili olmakla birlikte, kısıtlı olanakların uygun şekilde kullanılması için ek gıdaların niteliği ve başlama zamanları konusunda yeterli bilgisi olmadığı görülen annelere danışmanlık verilmesi yararlı olacaktır. Anneler bebek beslenmesi konusunda sağlık çalışanları tarafından birebir görüşmeler ve basılı materyallerle eğitilmeyi tercih etmektedir. P., Özkan Sınav H.Background:Carbohydrate counting (CC) is considered as a medical nutrition therapy method especially used in Type 1DM (T1DM) patients in basic and advanced levels. Limited number of studies reported that basic CCimproves glycemic control in T2DM patients, and there is need for further studies.Research questions:How the basic CC effects HbA1c levels in T2DM patients using oral antidiabetic drugs, when presented as acomponent of nutrition education by primary care physicians.Method:The study hereby is a non-randomized controlled clinical trial. T2DM patients, aged between 18-75 years,HbA1c levels between 6.5%-10.8% using oral antidiabetic medication, were followed for three months in twogroups; one intervention (IG) (n=33) and one control group (CG) (n=45), in two different Primary CareCenters. General diabetes monitoring principles were applied in both groups; basic CC training was givenadditionally to the IG. The training consisted of three sessions lasting 40 minutes each. An informative CCbooklet was provided to both groups. Beside sociodemographic questionnaire, a healthy nutrition informationscale was applied to all of the participants; HbA1c, other laboratory and anthropometric measurements wereperformed at the beginning and at the 3rd month of the study.Results:The mean age value was 54.58±9.69 (28-73). Male/female distribution was (IG:57.6%/42,4%,CG:53.3%/46,7%). At the third month, HbA1c value decreased within the IG from 7.7% to 7.2% (p:0.002),from 7.49% to 7.46%, in the CG (p:0.851). Comparing two groups, the decrease in HbA1c in the IG washigher than in the CG (-0.5, -0.02, p: 0.018).
