    Atypical presentation of medication-related osteonecrosis of the jaw in a young male patient undergoing chemotherapy for acute lymphoblastic leukemia

    This case report aims to highlight the importance of close oral monitoring before and during chemotherapy in patients with B-cell acute lymphoblastic leukemia (B-ALL) to prevent medication-related osteonecrosis of the jaw (MRONJ). A 22-year-old male patient diagnosed with B-ALL who developed MRONJ during chemotherapy, despite not receiving bisphosphonates or denosumab, was presented. The patient\"s medical history, treatment regimen, clinical examination findings, and treatment outcomes were reported. The patient developed maxillofacial cellulitis during chemotherapy, leading to a diagnosis of stage 3 MRONJ. Conservative treatment was initiated, resulting in the reversion of the disease to stage 1 and stabilization of the progression. The patient achieved remission and underwent prophylactic cranial radiotherapy. This case report presents a unique case of MRONJ in a young patient without prior use of antiresorptive agents or denosumab. It highlights the need for close oral monitoring in patients with B-ALL before and during chemotherapy, as MRONJ can develop in the absence of these medications.

    A randomized controlled trial comparing conventional and piezosurgery methods in mandibular bone block harvesting from the retromolar region

    Abstract Background Although piezosurgery is now commonly used for various applications in maxillofacial surgery, its advantages over conventional rotary instruments in terms of postoperative edema, ecchymosis, postoperative morbidity, and prolonged osteotomy time have been questioned. Materials and methods This study aimed to compare the efficiency, postoperative morbidity, and complication rates of piezosurgery and conventional methods in harvesting autogenous ramus grafts. In this randomized controlled trial, 21 patients (32 sides) underwent autogenous graft harvesting from the ramus area, with 16 sites treated using piezosurgery and 16 using the conventional method. The primary outcomes measured were osteotomy time, total operation time, and postoperative morbidity. Complication rates were also evaluated. Results The final analysis encompassed 19 patients, accounting for a total of 30 donor sites, following the exclusion of two patients who were unable to attend the scheduled follow-up visits. A total of 19 patients (30 donor sites) were included in the final analysis. No statistically significant difference was found in the mean osteotomy time between the piezosurgery group (mean: 10.35, SD: 2.74 min) and the conventional group (mean: 8.74, SD: 2.74 min) (95% CI: -3.67 to 0.442, p = 0.119). The total operation time, postoperative pain, and swelling were not significantly different between the two groups (p > 0.05). The complication rates, including wound dehiscence and inferior alveolar nerve exposure, were similar in both groups. Conclusions Piezosurgery can be safely used for harvesting autogenous ramus grafts and does not increase osteotomy or total operation time compared to the conventional method. The postoperative morbidity and complication rates were also similar, indicating that both techniques can be effectively employed in clinical practice.     Evaluating the influence of the mandibular canal trajectory on the duration of postoperative paraesthesia in patients undergoing inferior alveolar nerve lateralization: a prospective cohort study

    This prospective cohort study aims to evaluate the influence of the mandibular canal trajectory on the duration of postoperative paraesthesia in patients undergoing inferior alveolar nerve lateralization (IANL). Twenty patients received a total of 50 dental implants, and their postoperative paraesthesia duration, implant success rate, and anatomical variables were assessed. All patients experienced temporary neurosensory disturbances postoperatively during the first week, but none reported permanent issues at the 12-month follow-up. The median paraesthesia duration was 120 days, and no significant differences were detected between genders, anaesthesia types, or patient satisfaction. No significant association was found between the mandibular canal trajectory and postoperative paraesthesia duration. The implant success rate was 100%, with all implants integrating successfully. Our findings suggest that IANL is a safe and effective method for dental implant placement in atrophic mandibles.

    Interprofessional approaches in research planning training: Multidisciplinary perspective in oral and dental health

    Objectives: This study evaluates the effectiveness of an interdisciplinary educational intervention in improving the perceptions of interdisciplinary education, research competency, and research anxiety among postgraduate students in various health disciplines. Materials and Methods: This prospective cohort study was conducted at Marmara University during November 2023. A two-day training program, “Multidisciplinary Approach to Oral and Dental Health” was conducted, involving educators and postgraduate students from dentistry, speech and language therapy, nutrition and dietetics. The program consisted of four modules, combining didactic training and workshops. Data were collected pre-post-intervention using validated scales: Interdisciplinary Education Perception Scale (IEPS), Research Competency Scale (RCS), and Research Anxiety Scale (RAS). Scores were analyzed using a t-test for pre-test and post-test calculations. In the findings, a p-value of <0.05 was considered statistically significant. Results: Initially, 30 participants were registered for the study, but the final analysis was conducted on 26 participants. The statistical analysis indicated significant improvements in the postintervention scores for both the IEPS and the RCS. The average score for IEPS increased from 86.65 before the intervention to 91.04 after (p=.028), and for RCS, it rose from 25.92 to 28.08 (p=.010). However, RAS showed no significant change, with scores averaging 30.85 before and 30.65 after the intervention (p=.710). Conclusions: The intervention effectively enhanced interdisciplinary education perception and research competency among participants, emphasizing the potential of short-term, structured multidisciplinary programs in postgraduate dental education.

    Evaluation of Water Displacement Method in Estimating Mandibular Ramus Autograft Volume

    Objective: This study aims to identify the most reliable method for measuring graft volumes comparable to those harvested from the ramus region using 3D-printed models. Methods: Using a cross-sectional design in an in vitro setting, CBCT images from 20 individuals who met the inclusion criteria for ramus grafting were examined. Volumetric evaluations were conducted on these images, and 3D-printed graft models were created. Two blinded raters assessed the graft volumes using the displacement method (with beakers of 10 cc, 25 cc, 50 cc capacity and a 100 cc biopsy cup) and the overflow liquid method (with beakers of 10 cc, 25 cc, and 50 cc capacity). The intraclass correlation coefficient and t tests were applied for statistical validation of intra- and inter-rater reliability. Results: High levels of both intra- and interrater reliability were observed, particularly for the 10 cc rise and overflow methods. These methods exhibited not only exceptionally high ICC values but also statistically meaningful p values. Furthermore, most of these methods strongly correlated and agreed with the CBCT measurements, except for the 50 cc overflow method, which showed significant divergence. Conclusion: The findings of this study validate the 10 cc beaker methods for reliable 3D printed ramus graft volume measurement and recommend a narrow-diameter syringe for optimal accuracy.     Comparison of Inferior Alveolar Nerve Block and Infiltration Anesthesia Techniques for Pain Control in Primary Mandibular Molar Extractions in pediatric patients

    Amaç: Bu çalışma, çocuk hastalarda süt mandibular molar diş çekimleri sırasında ağrıyı yönetmede inferior alveolar sinir bloğu (IASB) ve bukkal infiltrasyon anestezisinin etkinliğini karşılaştırmayı amaçladı. Gereç ve Yöntemler: Bu prospektif gözlemsel çalışma Marmara Üniversitesi Diş Hekimliği Fakültesi’nde, rutin süt molar diş çekimi için sevk edilen çocukları kapsıyordu. Katılımcılar IASB ve infiltrasyon anestezi gruplarına ayrıldı. Çalışmanın birincil sonuç ölçütü, görsel analog skala (VAS) kullanılarak ölçülen ağrı şiddetiydi. Bu çalışmanın kovaryantları anestezi grubu, kalan diş kökü uzunluğu ve rezorpsiyon evresiydi. Tanımlayıcı istatistikler demografik ve klinik özellikleri özetledi. Mann-Whitney ve Kruskal-Wallis testleri, gruplar ve rezorpsiyon evreleri arasındaki VAS skorlarını karşılaştırırken, Pearson korelasyon ve çoklu doğrusal regresyon analizleri, ağrı şiddetinin ilişkilerini ve belirleyicilerini değerlendirdi. Sonuçlar: Katılımcıların ortalama yaşı 8.3 yıl (SD = 1.8) idi. Cinsiyetler, anestezi grupları veya rezorpsiyon evreleri arasında VAS skorlarında anlamlı bir fark bulunmadı. Korelasyon analizi, ağrı şiddeti ile kalan diş köklerinin yüzdesi arasında anlamlı bir ilişki göstermedi. Çoklu doğrusal regresyon analizi, hiçbir belirleyicinin ağrı şiddetini anlamlı şekilde açıklayamadığını gösterdi. Sonuç: İnfiltrasyon anestezisi, çocuklarda süt mandibular molar diş çekimleri sırasında ağrıyı yönetmede en az IASB kadar etkilidir. Bulgular, daha basit uygulanabilirliği ve daha az komplikasyonları göz önüne alındığında, bukkal infiltrasyon anestezisinin IASB'na geçerli bir alternatif olarak kullanılabileceğini desteklemektedir.
