    Determining T and B Cell development by TREC/KREC analysis in primary immunodeficiency patients and healthy controls

    T cell receptor excision circles (TRECs) and kappa-deleting excision circles (KRECs) are DNA fragments potentially indicative of T and B cell development, respectively. Recent thymic emigrants (RTEs) are a subset of peripheral cells that may also represent thymic function. Here, we investigated TREC/KREC copy numbers by quantitative real-time PCR in the peripheral blood of patients with primary immunodeficiencies (PIDs, n = 145) and that of healthy controls (HCs, n = 86) and assessed the correlation between RTEs and TREC copy numbers. We found that TREC copy numbers were significantly lower in children and adults with PIDs (P < .0001 and P < .002, respectively) as compared with their respective age-matched HCs. A moderate correlation was observed between TREC copies and RTE numbers among children with PID (r = .5114, P < .01), whereas no significant correlation was detected between RTE values and TREC content in the HCs (r = .0205, P = .9208). Additionally, we determined TREC and KREC copy numbers in DNA isolated from the Guthrie cards of 200 newborns and showed that this method is applicable to DNA isolated from both peripheral blood samples and dried blood spots, with the two sample types showing comparable TREC and KREC values. We further showed that RTE values are not always reliable markers of T cell output. Although additional confirmatory studies with larger cohorts are needed, our results provide thresholds for TREC/KREC copy numbers for different age groups.

    Effects on Turkish ice cream employees' musculoskeletal pain of a physical activity and ergonomics improvement program in the workplace

    Objectives. This study aimed to investigate the effects of a physical activity and ergonomics improvement program (PAEIP) on employees' musculoskeletal pain. Methods. The sampling group included 120 employees who met the criteria and were randomly assigned to the intervention group (n = 57) and the control group (n = 63). The visual analog scale, the Nordic musculoskeletal survey and rapid upper limb assessment (RULA) were used for collecting data. Results. When pre-test and post-test pain scores of the intervention group were compared, the average post-test scores were significantly lower in the neck, shoulder, wrist and lower back regions than the pre-test scores. When the intervention and control group RULA scores were compared, in the intervention group the proportion of employees found in an 'acceptable posture' level was 0% in the pre-test and 5.3% in the post-test, and in the same group the proportion of employees in the 'further investigation and change may be needed' level was 31.6% in the pre-test and increased to 61.4% in the post-test. Conclusion. As a result, the PAEIP has been found effective in reducing the ergonomic risks associated with upper extremities and rate or intensity of musculoskeletal pain.

    Özne ve nesne tarzında birbirinden tamamen izole edilebilir reel iki farklı var olandan bahsedilebilir mi yoksa böyle bir ayrım yalnızca soyutlamadan mı ibarettir? Şayet birbirinden ontolojik statüleri bakımından iki farklı var olan olarak özne ve nesne ayrımı söz konusu ise bunların ilişkisinde belirleyici olan hangisidir? Şayet bunlar soyutlama ise soyutlama öncesi durum nedir? Bu çalışmada; insanın özünü, “düşünen olmak”lığın, “bilinçli olmak”lığın tesis ettiğini iddia eden özne merkezli metafizik düşünceye karşın, insanın özünün “dünyada olmak”lığın teşkil ettiğini iddia eden Heideggerci fenomenolojik eleştirinin temel tutamak noktaları, yukarıdaki iki soru bağlamında ele alınmaktadır.     A Non-invasive method for assessment of intravascular fluid status: Inferior vena cava diameters and collapsibility index

    Objective: To evaluate the correlation between central venous pressure (CVP) and inferior vena cava (IVC) diameters measured by ultrasonography (Ultrasound) in critically ill patients. Methods: Intubated critically ill patients were enrolled. The CVP values were measured using a U-tube manometer and were compared to the IVC diameters and collapsibility index, which were measured by bedside Ultrasound. Patients younger than 18 years old, who were not intubated, who had an abdominal pressure greater than 12 mmHg, and/or who were admitted for trauma were excluded from the study. Results: Eighty three patients with a mean age of 73.6 +/- 11.2 years were enrolled. The most common diagnosis was sepsis (21 patients, 25.30%). IVC inspiration measurements were statistically significantly correlated with CVP measurements (p0.05, r: 0.1). IVC collapsibility measurements showed a negative correlation with CVP measurements (p<0.01, r: 0.68). Conclusions: There is a strong correlation between CVP and IVC diameters and the collapsibility index. This is a new formula for evaluating CVP, based on our statistical analyses.

    Effect of Partial Oxygen Supply on Mitochondrial Electron Transport System During Complete Cardiac Ischemia

    Abstract During complete ischemia we assessed myocardial utilization of the small amount of oxygen available. We also determined whether blood cardioplegia has any advantage over crystalloid cardioplegia in this setting. Patients with preserved left ventricular myocardial function and without anterolateral wall infarct or aneurysm were included to the study. Intermittent cold blood and crystalloid cardioplegia were used in 10 patients (group BC) and 9 patients (group CC), respectively. From myocardial biopsies, obtained before and after ischemia, complete electron transport system (ETS) enzyme activities (NDH, SDH, NCCR, SCCR, and COX) and lactate content were analyzed. Biochemical and hemodynamic analyses also were done. Myocardial and blood temperatures were monitored. Ischemic time was longer in group CC (p < 0.05). There were no important differences in biochemical and hemodynamic variables between the two groups. In addition, there was no difference in NDH and SDH activities as well as COX/SCCR and COX/RS‐NCCR ratios between the two groups before and after ischemia. After Ischemia, RS‐NCCR in group CC and SCCR and COX activities in both groups were lower than the control. For all enzymes, activity change ratios were not different between groups. Myocardial lactate content was increased in both groups after ischemia. However, the increase in group BC was less (p < 0.01). Based on our findings, we believe that the superiority of blood cardioplegia over crystalloid cardioplegia does not depend on oxygen content, but on other factors such as buffering and free oxygen radical scavenger effects among others. However, with the warm and continuous blood cardioplegia technique, oxygen content might be more important. Copyright © 1985, Wiley Blackwell. All rights reserved
